Concomitant products included TRAZODONE for an unknown indication. On 24-Feb-2021, the patient received first dose of mRNA-1273 (Moderna COVID-19 Vaccine) (Intramuscular) 1 dosage form. On 24-Mar-2021, received second dose of mRNA-1273 (Moderna COVID-19 Vaccine) (unknown route) dosage was changed to 1 dosage form. On 27-Dec-2021, received third dose of mRNA-1273 (Moderna COVID-19 Vaccine) (unknown route) dosage was changed to 1 dosage form. On 29-Dec-2021, after starting mRNA-1273 (Moderna COVID-19 Vaccine), the patient experienced VACCINATION SITE REACTION (Covid arm). On 08-Jan-2022, the patient experienced URTICARIA (they started to have and are ongoing ,sever hives, a rash and itchiness), PRURITUS (they started to have and are ongoing ,sever hives, a rash and itchiness) and RASH (they started to have and are ongoing ,sever hives, a rash and itchiness). The patient was treated with LORATADINE, PSEUDOEPHEDRINE HYDROCHLORIDE (BENADRYL 24 D) at an unspecified dose and frequency and CETIRIZINE HYDROCHLORIDE (ZYRTEC ALLERGY) at an unspecified dose and frequency. On 31-Dec-2021, VACCINATION SITE REACTION (Covid arm) had resolved. On 24-Mar-2021, received second dose of mRNA-1273 (Moderna COVID-19 Vaccine) (unknown route) dosage was changed to 1 dosage form. On an unknown date, the patient experienced VACCINATION SITE REACTION (Covid arm). The patient was treated with DIPHENHYDRAMINE HYDROCHLORIDE (BENADRYL [DIPHENHYDRAMINE HYDROCHLORIDE]) at an unspecified dose and frequency and CETIRIZINE HYDROCHLORIDE (ZYRTEC [CETIRIZINE HYDROCHLORIDE]) at an unspecified dose and frequency. In April 2021, VACCINATION SITE REACTION (Covid arm) had resolved. Height of the patient was mentioned as 411 and 3/4. It was reported that the patient experienced Covid arm eight days after the first and second dose and it lasted for two weeks. Symptomtext

About 2 weeks after the second Moderna shot, patient developed dry cough and noticed a weight loss. Patient went to Dr. to report condition. Dr. did bloodwork, chest x-ray, and abdominal ultrasound. All came back negative. Patient has subsequently started taking Ensure, which has stabilized weight, but continues to have digestive issues, which is mostly stomach gas that seems to come mostly severely when sitting down. Patient takes 1-3 Tums a day to deal with gas, but has not returned to Dr. as the situation seems manageable, though somewhat distressing as gas has never been a problem with patient before. The dry cough, the wet loss of about 10 pounds, and the stomach gas, all seemed to develop after the second Moderna shot. The patient did test positive to Covid in November, 2020, and had a mild case that did not require treatment.
