74 year old white female pt. received second dose of Moderna-COVID-19 vaccine on 3/4/21. Pt. called to report severe leg rash that started at the end of March 2021 that has been ongoing since and has spread to other parts of her body. Currently rash is located on chest, arms, legs, groin, and breasts. Patient states she was diagnosed with Hashimoto's disease 8 or 9 years ago. She's been taking Levothyroxine 75 mcg daily for nine years, Caltrate 600 + B3 for "many years", vitamin D3, Vitamin C 1000 mg for 20 years, and Omega 3-6-9 for about a year. She also reports she began taking Atorvastatin 40 mg daily on 4/9/21. Pt. reports an allergy to Wellbutrin that causes pruritis. Pt. states Monistat cream also causes pruritus. Pt. denies any illness at time of vaccination and prior to. Patient encouraged to seek medical treatment for symptoms by this RN.
