collapsed/From toilet to his bed where he essentially collapsed; could not stand; couldn't walk; This spontaneous case was reported by a consumer and describes the occurrence of LOSS OF CONSCIOUSNESS (collapsed/From toilet to his bed where he essentially collapsed), DYSSTASIA (could not stand) and GAIT DISTURBANCE (couldn't walk) in an 84-year-old male patient who received mRNA-1273 (Moderna COVID-19 Vaccine) (batch nos. 065K21A, 032F21A, 001A21A and 0011J20A) for COVID-19 vaccination. Concurrent medical conditions included Dementia and Walker user. On 12-Feb-2021, the patient received first dose of mRNA-1273 (Moderna COVID-19 Vaccine) (Intramuscular) 1 dosage form. On 11-Mar-2021, received second dose of mRNA-1273 (Moderna COVID-19 Vaccine) (Intramuscular) dosage was changed to 1 dosage form. On 04-Nov-2021, received third dose of mRNA-1273 (Moderna COVID-19 Vaccine) (Intramuscular) dosage was changed to 1 dosage form. On 11-Apr-2022, received fourth dose of mRNA-1273 (Moderna COVID-19 Vaccine) (Intramuscular) dosage was changed to 1 dosage form. On 12-Apr-2022, the patient experienced LOSS OF CONSCIOUSNESS (collapsed/From toilet to his bed where he essentially collapsed), DYSSTASIA (could not stand) and GAIT DISTURBANCE (couldn't walk). At the time of the report, LOSS OF CONSCIOUSNESS (collapsed/From toilet to his bed where he essentially collapsed), DYSSTASIA (could not stand) and GAIT DISTURBANCE (couldn't walk) outcome was unknown. The concomitant medication was not reported. The treatment medication was not reported. The patients wife reported that her husband received 4 doses of the Moderna covid19 vaccine by intramuscular injection. He had no problems after receiving the first three doses. He had a history of dementia and normally ambulates with a walker. Today as reported he was using the toilet, but could not stand from the toilet on his own. He normally was able to do this quickly, but it took him about 20 minutes to stand from the toilet today. He was eventually able to make it from the toilet to his bed where he essentially collapsed. This occurred about an hour ago as reported. He had not taken any medications or treatments for his symptoms. The contact had called his neurologist and PCP leaving messages. The patients wife was not concerned for stroke or heart attack at this time.

29 days after patient's second Moderna dose, she had sudden onset significant pain where she couldn't stand or walk, her left leg became swollen twice it's size, and blue. She went to the ER with no treatment. About 5-6 hours after discharged she returned with her leg turning purple and uncontrollable pain. She was diagnosed with venous thromboembolis and transferred to a larger facility where she underwent lytic therapy and venous angioplasty. She is having to go for venous stenting in the next two weeks. She continues to have pain in her leg, chronic fatigue and weakness, as well as being lightheaded with a stable BP (no orthostasis). She has been diagnosed with anemia since vaccination and is being worked up for that.

Did not want to receive the second dose; Horrible Headache; Hypokalemia; Chest Pain; Chest Pressure; Felt like I was hit by a train; This spontaneous case was reported by an other health care professional and describes the occurrence of HYPOKALAEMIA (Hypokalemia) in a 52-year-old female patient who received mRNA-1273 (Moderna COVID-19 Vaccine) (batch no. 0011J20A) for COVID-19 vaccination. The occurrence of additional non-serious events is detailed below. No Medical History information was reported. On 23-Dec-2020, the patient received first dose of mRNA-1273 (Moderna COVID-19 Vaccine) (Intramuscular) 1 dosage form. On 23-Dec-2020, the patient experienced HYPOKALAEMIA (Hypokalemia) (seriousness criterion medically significant), CHEST PAIN (Chest Pain), CHEST DISCOMFORT (Chest Pressure), FEELING ABNORMAL (Felt like I was hit by a train) and HEADACHE (Horrible Headache). On an unknown date, the patient experienced INTENTIONAL DOSE OMISSION (Did not want to receive the second dose). The patient was treated with POTASSIUM (intravenous) for Hypokalemia, at an unspecified dose and frequency. On 26-Dec-2020, FEELING ABNORMAL (Felt like I was hit by a train) had resolved. At the time of the report, HYPOKALAEMIA (Hypokalemia), CHEST PAIN (Chest Pain), CHEST DISCOMFORT (Chest Pressure) and HEADACHE (Horrible Headache) outcome was unknown and INTENTIONAL DOSE OMISSION (Did not want to receive the second dose) had resolved. DIAGNOSTIC RESULTS (normal ranges are provided in parenthesis if available): On 23-Dec-2020, Blood pressure measurement: high (High) blood pressure was "through the roof. The action taken with mRNA-1273 (Moderna COVID-19 Vaccine) (Intramuscular) was unknown. For mRNA-1273 (Moderna COVID-19 Vaccine) (Intramuscular), the reporter did not provide any causality assessments. Concomitant product use was not provided by the reporter. Day 1, I was fine. The next day, I felt like I had a fever but it wasn't showing per thermometer. I had shooting pain anywhere I scratched too hard. Two days later I still had nerve pain. Nine days after vaccine, I noticed the flat patch on my arm at vaccine site. Really wasn't itchy. Friday, the flat patch was elevated and swelling. Upper lip was itchy and my face was asymmetrical. I am allergic to most tree nuts, but not all. I can usually eat pecans and I did have one. The patch grew from Thursday, and the largest it got was 5 3/4 inches. I never got a fever during all of this. I took Pepcid along with ibuprofen for treatment.

Itching at injection site that began 7days after the injection. A red puffy area developed the next day. It is oval shaped about 2 inches by 1.5 inches and is itchy. I have used one application of topical Benadryl and it has not helped.

Product leaked out and dripped down her arm; Product leaked out and dripped down her arm; A spontaneous report was received from a healthcare facility staff member who was a 44-year-old, female patient who received Moderna's COVID-19 vaccine (mRNA-1273) and experienced the product leaked out and dripped down her arm. The patient's medical history was not provided. Concomitant product use was not provided. On 30 Dec 2020, the patient received her first of two planned doses of mRNA-1273 (Lot number: 0011J20A) intramuscularly in the left arm for prophylaxis of COVID-19 infection. On 30 Dec 2020, some of the product leaked out of the syringe and dripped down the patient's arm during vaccination. She denied experiencing any side effects. Treatment information was not provided. Action taken with mRNA-1273 in response to the events, product leaked out and dripped down her arm, was not provided.
