03/15/2021- Flu like symptoms, low grade fever, body aches, fatigue, chills. Lasted for 12 hours. 03/30/2021 About two weeks after vaccine I had arm rash, swelling and inflammation. Started in April 04/15/2021, hands achy, hard to move fingers, hard to grasp anything, I still feel it in my thumbs.

NOV 2021 I NOTICED A BLOTHY REDNESS ON MY RIGHT BREAST?. I HAD A MAMMO AND SONO WHICH WERE NEG?.FEB 2022 I NOTICED ANOTHER AREA ON THE SAME BREAST. I SAW A GP. HAD MAMMO AND SONO. SAW A BRESRT SURGEON. ALL NEG. IN JUNE 2022 I SAW A DERMATOLOGIST WHO BIOPSIED THE SECOND SITE. PATHOLOGY WAS MORPHEA. WHICH INDICATED AN POSSIBLE AUTO IMMUNE RESPONSE. I SAW A BREAST SURGEON WHO SUGGESTED IT WAS A VACCINE REACTION. I HAVE NOW HAD ALL 4 VACCINES. THE REDNESS REMAINS.

Symptomtext

There are no adverse effects known at this time. Patients who came on 2/11/2021 were due back on 3/11/2021. The system was down and a reminder email could not be sent. We asked the help desk to for a list of patients from 2/11/2021 to issue a reminder to patients. An email was sent out to all patients on the list. Patient's wife received the email. She called our office to ask about the 2nd dose email and a time to come in for the vaccine. Representative from our office registered patient from the information we received on the spreadsheet from the help desk. She dod not see that the report listed patient as a waitlist patient and was registeredfor his 2nd dose at the clinic for a 2nd dose of Moderna. Patient received a dose of Moderna on 3/11/2021 and upon checking out with the physician onsite it was discovered that he had not received his 1st dose of Moderna at our office on 2/11/2021 but had received his 1st COVID-19 vaccine on 2/23/2021 at a County site and the vaccine he received was Pfizer. Patient received a mixed series within 16 days of each other. He has no known adverse effects at his time. We spoke with the patient and his wife at the close of our clinic explained the error which occurred and let them know we would be filing a report with VAERS. We have made changes to our clinic process and have 3 check points and reviews of vaccination cards in additional to our normal process for 2nd dose patients.
