Covid shortness of breath. Patient states that she was diagnosed with Covid on Thursday. Patient states she was admitted and discharged on Saturday. Patient states that she is going to get antibiotic infusion therapy when they stated that she was saturating in the mid 80s and then had outside oxygen ordered. Patient states today she has worsening dyspnea. Patient states she is vaccinated. Patient lost taste and smell. Patient was saturating 77% on room air and 90% on nonrebreather, patient was placed on high flow nasal cannula.

- ?Moderna Covid-19 vaccine EUA? - 1/29/2021 @ 3 pm: received first dose of Moderna Covid-19 vaccine in the deltoid region of left arm (she did not receive a second dose because of the adverse reaction to the first dose of the vaccine) - First 3 days after the vaccine: normal local pain - On Monday morning February 1st 2021: left arm became highly sensitive to touch with tingling and sense of severe fatigue and weakness, At night, pins and needles and muscle twitches. This continued with variable intensity. - Friday February 5th : Started to feel Left leg strain as if pulled muscle, and at night twitches and prickling sensation in left lower limb - Saturday February 6th at night: right lower limb started to feel muscle strain and numbness - Sunday morning February 7th, we went to ER, the ER physician consulted the neurologist who ordered blood test for anti GM1 antibodies and scheduled a neurology consultation - Thursday February 12: neurology consultation (Video) with Dr., who advised to watch progress till the end of the month of February - Left arm started to feel muscle strain and numbness too - Muscle twitches continued in the 4 limbs with apparent weakness that affect her daily activities - Tuesday March 2nd Talked to the neurologist again, who recommended some blood tests and MRI of the neck - As of today March 3rd , still feeling weak arms and legs with muscle twitching specially at night, no problem with sensation. She became very emotional with high anxiety level

slight rash on my face on that day/fades a little bit during the day but comes back; Pretty red on my face/It does not go away.; acne condition where I have little pimple/across the bridge of my nose, under my eyes, under my right jaw and on my cheek area; This spontaneous case was reported by a consumer and describes the occurrence of RASH (slight rash on my face on that day/fades a little bit during the day but comes back), ERYTHEMA (Pretty red on my face/It does not go away.) and ACNE (acne condition where I have little pimple/across the bridge of my nose, under my eyes, under my right jaw and on my cheek area) in an 83-year-old female patient who received mRNA-1273 (Moderna COVID-19 Vaccine) (batch nos. Lot# 029L20A and Lot# 006M20A) for COVID-19 vaccination. Concomitant products included ESCITALOPRAM OXALATE (LEXAPRO) for an unknown indication. On 22-Jan-2021, the patient received first dose of mRNA-1273 (Moderna COVID-19 Vaccine) (unknown route) 1 dosage form. On 19-Feb-2021, received second dose of mRNA-1273 (Moderna COVID-19 Vaccine) (unknown route) dosage was changed to 1 dosage form.
