Symptomtext
Patient c/o feeling dizzy, faint and temporal pressure post 10 minutes of 1st vaccination (Moderna/HRSA). Patient has a medical history of hypertension, Gerd and per husband CSF. Allergies: NKDA. Patient stated she did take her blood pressure medication as prescribed @ noc. Patient vitals were monitored. @ 11:25 AM BP 156/39 HR 96 spo2 97% RA. A/OX3. Patient became anxious and Temporal pressure increased. Ambulance called and patient transferred to Hospital.
