Please note: The vaccination record I received with lot number is very difficult to read!!! My first vaccination was 12/31/20 and lot number written down but hard to read is (U37K2OA but could be V37K20A) The second shot was 1/27/21 and lot number written down could be (0412OA, or could read O912OA!) I had symptoms of a Pulmonary embolism while cross country skiing March 18 and officially diagnosed with PE on March 22, 2021. I was tested for Covid 19 on March 23, 2021 with negative results. I had been reporting my side effects (which were none) for the previous 6 weeks following the second vaccination. I did have cramping like feeling in my right calf following a x-country ski weekend about 4 weeks prior to the actual PE (2/22/21) but it appeared to resolve after about 7-10 days

Barely any soreness on the injection site after 3rd shot; This spontaneous case was reported by a consumer and describes the occurrence of VACCINATION SITE PAIN (Barely any soreness on the injection site after 3rd shot) in a 70-year-old female patient who received mRNA-1273 (Moderna COVID-19 Vaccine) (batch nos. 037F21A, 04120A and 030M20A) for COVID-19 vaccination. No medical history was provided by the reporter. Previously administered products included for Product used for unknown indication: influenza (flu shot) on 14-Sep-2021. Past adverse reactions to the above products included No adverse event with influenza. On 14-Jan-2021, the patient received first dose of mRNA-1273 (Moderna COVID-19 Vaccine) (unknown route) 1 dosage form. On 11-Feb-2021, received second dose of mRNA-1273 (Moderna COVID-19 Vaccine) (unknown route) dosage was changed to 1 dosage form. On 17-Sep-2021, received third dose of mRNA-1273 (Moderna COVID-19 Vaccine) (unknown route) dosage was changed to 1 dosage form. On 17-Sep-2021, after starting mRNA-1273 (Moderna COVID-19 Vaccine), the patient experienced VACCINATION SITE PAIN (Barely any soreness on the injection site after 3rd shot). At the time of the report, VACCINATION SITE PAIN (Barely any soreness on the injection site after 3rd shot) outcome was unknown. tender to the touch; injection site is like run in fever; swollen; red; A spontaneous report was received from a (Consumer) concerning a 48 Years-old female patient who received Moderna's COVID-19 vaccine (mRNA-1273) and experienced events really swollen, red and tender to touch and the injection site is like run in fever. The patient's medical history was not provided. Thyroid pill concomitant medications were reported. On 13,jan,2021, prior to the onset of the events, the patient received their first of two planned doses of mRNA-1273 (lot/batch: 04120A)on left arm(non dominant) intramuscularly in the Anatomical location for prophylaxis of COVID-19 infection. On 13,jan,2021 she experienced swollen,red and after two days its been looking worse tender to very warm to touch and injection site is like run in fever.
