Narrative: 58 yo male with medical co-morbidities including HTN, HLD, DM2, OSA, depressive disorder, alcohol use disorder in remission (no hx of complicated withdrawals), morbid obesity, MAFLD with hx of decompensated cirrhosis and hepatic encephalopathy, OA, left distal femur s/p IM rod/left kneecap fx s/p ORIF (1986, 2/2 automobile accident), eczema hospitalized for inability to care for himself and reported hypoxia to SpO2 mid-80s. Admission PLT 195 (on 4/24), dropped from 110 on 5/3 to 6 on 5/4. Concern for HIT vs. drug-induced thrombocytopenia vs. other. COVID-19 Pfizer vaccine given 4/26. Patient was on enoxaparin for DVT prophylaxis from 4/27-5/4; HIT screen on 5/4 as negative. 5/5 labs include: haptoglobin 107, fibrinogen 401, lactate dehydrogenase 219, PLT 5. Patient given 2 platelet transfusions on 5/5 and 5/6 and dexamethasone 40mg daily x 4 days with further hematology work-up.
