Pt was vaccinated with 1st dose of Pfizer. Pt was waiting in post vaccination observation when at 1500 she stated of itchy hands. Client brought down to floor from bleachers. 1506 Client endorsed itchiness travelling up to forearm and "tingling to mouth. Aid called and arrived to evaluate Pt. 1510 Client care transferred to Aid for additional evaluation and care. 1530. Client to go home per aid. No further symptoms and symptoms decreased. Pt d/c home per EMTs. Client educated on safety and when to call 911 or go to ED. VS 1503 HR 92 O2 sat 99% on RA VS 1510 HR 84 O2 sat 98% on RA VS 1520 HR 77 O2 sat 99% on RA

Began experiencing muscle pain in my legs on May 4, along with a large bruise on my upper thigh. I have had 5 flare ups of these painful eruptions of bruises. Doctors believe it is autoimmune response. Presenting as panniculitis or other dermal inflammatory reaction.

Symptomtext

Shortly after the 1st dose, I developed moderate/severe hip pain, bilaterally. Hip pain keeps me up at night, it disturbed my sleep. I can't sit for a prolonged time due to hip pain. It makes walking difficult, especially up and down the stairs. The pain didn't go away 4 months later and it didn't get better. My 1st vaccination was in April 2021; it is August 2021, when I write this report and hip pain is still present.

Per safety officer to this writer: Pt DOB 10/2/1972. Pt had first dose of Pfizer vaccine, began feeling dizzy and lightheaded immediately after. Pt stated "its hard to speak". Pt given juice and water. AID called to evaluate Pt. Pt requested interpreter. Pt states she had not been feeling well the last several days, has low grade fever and allergies to ibuprofen (itchy and trouble breathing). 10:19: Pt was evaluated by EMTs, released from EMT care. Pt rested on cot in observation area. RN assisted in calling Pt ride to pick Pt up. Pt reports hx of migraines. 10:46: a friend/relative arrived to pick Pt up and drive her home. 11:14: Pt walked out with significant other to car. VS 1010 BP 120/80 HR 110 RR 24 Temp 99.6F temporal O2 sat 100% on RA VS 1023 HR 85 RR 20 Temp 99.9F temporal O2 sat 100% on RA VS 1033 Temp 98.5F temporal

Pt felt dizzy and laid down on the floor. He stated he typically get's dizzy with needles. Pt was moved to a cot and assessed by MD. Mother reported that pt gets dizzy with blood draws and always lies down. Pt has autism, seizure disorder, and doesn't do well in new settings. While lying down pt complained of dizziness. Release to home with mother.

Adverse Event: Guest checked in and was screened according to protocol and denied receiving any injections within the last 14 days. When guest was checking out, it was discovered that he had received the Pfizer Vaccine on 4/29/2021 (Lot #ER8729) - Left Deltoid. Treatment: Guest assessed for adverse side effects - none observed. On-Call medical provider notified - advised to schedule guest for their second dose according to vaccine schedule. Guest educated on the risks and benefits of his actions and instructed to go the the nearest emergency room if he experienced any untoward events. Outcome: Guest educated. No adverse side effects observed or reported. Guest scheduled for second dose according to Pfizer Vaccine Schedule. Guest encouraged to report this event to his primary care provider.

PT DOB 4/14/1992 recieved 1st dose of Pfizer vaccine today. Per Safety officer report to this writer: Pt stated he had a smoothie this morning. Pt has needle anxiety from other shots. Pt states blurred vision but after laying down, Pt states the blurred vision resolved. Pt was given juice. EMS arrived to evaluate Pt. After 10 min, Pt condition resolved further. Pt cleared by EMS and was asked to wait another 15 min. Pt waited another 15 min and ambulated out of facility with steady gait. VSs per EMS 0918 BP 82/palp HR 56 O2 sat 99% on RA VSs per EMA 0924 BP 102/palp HR 76 RR 14 O2 sat 100% on RA
