Symptomtext

Patient lost consciousness two times, one on 06/28 and the second one on 07/15. First time patient felt dizzy/nauseous, blurred vision (loss of color), loss of hearing, and then loss of consciousness. The patient woke up 5 minutes after. Patient felt slight confusion with no adverse effects after. Second time patient almost lost consciousness three times before finally losing consciousness. First time patient loses consciousness for less than 3 seconds before waking up. 3 minutes later patient felt dizzy before losing consciousness for less than 3 seconds. 6 minutes later patient felt dizzy before losing consciousness for less than 3 seconds. Less than 1 minute later patient felt dizzy and nauseous before losing consciousness for 1 minute. After waking up patient felt slight confusion with no adverse effects.

Symptomtext

Vaccine was administered on 7/7/2021. On 7/9 pt woke up feeling like something heavy was sitting on her chest and said it was hard to catch her breath. She would get winded walking up a flight of stairs. Symptoms worsened throughout the day and was taken to the ER around 2am on 7/10/2021. Upon arrival at the ER, she rated her chest pain a 6/10. A CXR and EKG were completed and returned normal. She was diagnosed with inflamed tissue around her sternum and told to continue to take Motrin at home.

Symptomtext

Within 24-hours I experienced stabbing pain in the Uterus/Abdominal area. It lasted approximately several hours. Dull pain lasted 24 hours. I then missed my period for 2 months. Since then I have been experiencing cramps when I get my period. I have never, in the history of my 34 years gotten cramps and my period is very regular. Missing a period is not normal for me and cramping is not normal. There was my menstrual cycle for the 34 years prior to my second shot and my menstrual cycle now. It has returned in regularity but I now have low grade cramping.

I did not do anything. I just toughed it out because I had work and did not have time to deal with it. I am sure no one is really going to care because I did not die but something happened and I want to understand or at least document it. I am uncertain how this will impact fertility. I am familiar with data suggesting there is no problem for the aggregate but I am not the aggregate.

After recieving the Covid 19 Pfizer EW0181 vaccinne at pharmacy I expeirienced icing on the injection site a hour later. During the same day i experieced icing on my legs. the next day i saw rashes spreading all over my legs as well as bruises. its been spreading to my chest next and arms. I went to the doctor and he prescribed Triamcinolone Acteonide and Hydroxyzine HCL25MG. I have tried benedryl, cortizon, and providone-lodine, and calimine lotion with no possive effect. The doctor instructed me to file this form with you and instructed me not to take the second dose
