Confusional state Lethargy Seizure like phenomena

Symptomtext

Pt. with hx of seizure activity with vaccination displayed seizure like activity approximately twenty minutes after second dose of vaccine. No LOC. Pt. experienced no respiratory distress during incident. Activity lasted 3 minutes. Pt. presented confused and lethargic after activity. EMS involved and initial vitals were stable. Pt. A&Ox4, states this is similar to seizure activity experienced within the last month and chronically. EMS released pt. to home, pt. exited facility in company of family who affirmed they were driving pt. home and would remain with pt. through the rest of the day.

Immediate post-injection reaction Loss of consciousness Muscular weakness

Symptomtext

This was reported to this writer. The patient lost consciousness immediately after the injection. Laid on floor by daughter. There was no head trauma. EMS was called and assumed care. Patient's legs were elevated. Laid on floor for 10 minutes. Conscious and able to answer questions from EMTs, with daughter interpreting. Patient was eventually assisted to a wheelchair and wheeled into the observation area. There was leg weakness with transfer. While in the post vaccination observation area, patient was talking, drinking water. Appeared stable. 15 minutes from EMT arrival, they left. Patient able to stand and walk with stable gait 45 minutes following the incident. Taken home by daughter.

Symptomtext

approximately 18 minutes after patient received vaccination she reporting feeling as though she couldn't breath. She continued speaking in a normal volume and tone without any signs of altered breathing. Color was appropriate and vitals stable. She requested and was given 10 ccs of pediatric Benadryl and observed for 1/2 an hour until she reported feeling normal.

Client became anxious 5 minutes following injection. Complained of short of Breath, being dizzy .R.N working with her and mom. in observation area, Assessed by EMT's. Vital signs stable. At 15 minute mark, EMT's evaluated client to be medically Stable and able to be discharged home.

Symptomtext

This description was reported to this writer. A few minutes after receiving the vaccine and sitting in the observation area, the patient's father told the nurse that his son was feeling lightheaded, was sweating and appeared pale. EMTs were notified and approached the patient. The patient was laid onto the ground and his legs were elevated. EMTs did an assessment and vital signs. Reported that the patient was hypotensive. Dad stated that his son had not eaten anything yet today. He was then given juice and a small candy bar which he ate. Patient was soon transferred to a wheelchair and taken to a nearby cot to lay down again. Dad stated this son does not have any allergies. Dad remained at son's side until released to go home by EMS. Patient ambulated unassisted toward the car.

Began experiencing muscle pain in my legs on May 4, along with a large bruise on my upper thigh. I have had 5 flare ups of these painful eruptions of bruises. Doctors believe it is autoimmune response. Presenting as panniculitis or other dermal inflammatory reaction.

Symptomtext

Adverse Event: Client reports severe headache, left flank pain, copious amounts of emesis observed, diaphoresis, change in level of consciousness, audible wheezing, beginning phases of posturing. Client spouse reports this is how she presents with previous episodes of anaphylaxis and seizure episodes. Treatment: Interventions were assisting guest to the medical area via wheel chair, cold packs, emesis bags, EMS activated. hydration with water, therapeutic conversation, epic pen administered x1, unable to obtain vital signs due to posturing. Outcomes: Wheezing resolved, increased LOC, consented to being transferred to Medical Center. Spouse at her side and daughter left together.

Pt DOB 1/4/1994. Pt had his first dose of Pfizer and was sitting in post observation. Per post observation staff to this writer: Pt began to jerk and slumped to his right side. Pt became diaphoretic, eyes were fixed but was able to answer all questions. Pt was laid onto cot feet elevated until Aid arrived. Guest reports he came from the gym and has not eaten today. Pt back to baseline and cleared to d/c home. Pt mother came to drive Pt home.

Patient received Pfizer COVID vaccine on 4/28/21 and 5/19/21. On 12/28/21, patient admitted with cholelithiasis and gallstone pancreatitis to undergo laparoscopic cholecystectomy and cholangiogram. Patient was found to be COVID positive upon admission, but asymptomatic from respiratory point of view throughout the hospitalization. Patient was discharged home on 12/30/21 after recovering from surgery.

Symptomtext

Patient received the Pfizer vaccine 28 days following the administration of the Moderna vaccine. The Moderna vaccine was given at a different location than this dose of Pfizer. Patient sat for the recommended 15 minutes for observation. He stated that he felt fine and left for home with his father. Both the patient and his father were told that getting one dose of each is not ideal but that there is no medical concerns associated with this. Told the potential side effects are the same for both vaccines. Told that there is no need for a second Pfizer. Will still be considered fully vaccinated in 2 weeks.
