Symptomtext
This patient received a total of 3 doses of Pfizer vaccine. The first dose was administered in February 2021 outpatient at a Pharmacy. The patient has been admitted with us for sometime and we were not aware the patient had already received a vaccine. We vaccinated this patient at a 3 week interval per the Pfizer dosing guidelines and it was realized that he already had received a dose when manual entry into the state portal was occurring. Patient has not experienced any medical adverse effects from the additional dose at this time.
