Symptomtext
Patient received an adult Pfizer booster vaccine in the left arm(Lot# FF88411, Expiration Date 01/03/22). While in the Observation Area client reported blurry vision and feeling light headed at approximately 1034. At that time patient also self administered own medications of Lorazepam and Tylenol as prescribed for symptoms she was experiencing. PHN's present in Observation Area. Patient was assisted to the anti-gravity chair, Blood Pressure 142/86, SaO2 98% on room air, Pulse 92, breathing unlabored, alert and oriented to person, place, time, and situation. Patient states, "blurry vision is getting better, a little." Patient denied reactions to COVID vaccine 1st and 2nd doses. 1040: Blood Pressure 142/88, SaO2 98% on room air, Pulse 82, breathing unlabored, no changes in mentation. Patient denies dizziness, admits to eating coffee and toast at approximately 0800. 1052: Blood Pressure 143/88, SaO2 99% on room air, Pulse 84, breathing unlabored, no changes in mentation. Patient states she "feels dizzy with high blood pressure." 1054: Patient states she "feels fine," PHN educates patient on signs and symptoms of anaphylaxis and to call 911 or go to nearest emergency department for any signs or symptoms, advises patient to follow up with primary care provider regarding blood pressure. Patient admits she has 2-3 episodes of dizziness daily and also has diabetes she no longer takes medication for. 1055: Patient leaves with husband for home.
