Symptomtext

Patient Developed for purpuric, non blanchable rash on both legs below the knee bilaterally. 24 hours after vaccine, patient developed muscle aches and headache, no fevers. rash developed 1-2 days after vaccine. no abdominal pain, no diarrhea, no joint pain. Patient described rash as itchy. Examination of rash in clinic most consistent with vasculitis vs Henoch-Sch?nlein purpura.

Patient was evaluated in clinic on 10/27/2021. Patient was noted to have a normal blood pressure in clinic. Blood work obtained on 10/27/2021 included a Comprehensive metabolic panel, ESR, and CBC w/ differential. Comprehensive metabolic panel showed normal kidney function BUN: 9 mg/dL, creatinine 0.68 mg/dL . Electrolytes were normal for age. Normal albumin 4.3 g/dL, Normal total protein 7.8 g/dL. Normal transaminases and bilirubin. 10/28/2021 Urinalysis was obtained which showed no blood, protein, or white blood cells. Photos were obtained on 10/27/2021 with parental and patient consent and sent securely to Hospital. Photos reviewed with dermatologist who reviewed case and photos securely. He thought that rash was most consistent with vasculitis, likely HSP which can occur after vaccines. Verbal check in with family on 11/3, Family reported that rash has not spread and remains unchanged on lower legs. No abdominal pain, joint pain, or changes with urination. Patient to have follow up for blood pressure assessment and urine dip on 11/17/2021.

History of acne, not taking any medications. History of vitamin D deficiency, unclear if she was taking Vitamin D supplement at time of vaccine.
