Guardian brought patient to a covid 19 pop up event at the Center. When verbally asked how old the child was, she reported 12 years old. She then filled out the paper work and reported her dob as date (which would have made the child 12 yo). Approximately 10 min after recieving the vaccine and she was waiting her 15 min., when I was search for the child in system, unable to find her I omitted the dob and her name came up with dob as other date (11 y/o). I inquired of Guardian if that was the correct dob, she just laughed and said yes it is. I informed her that would the child 11 y/o instead of 12. She reported yes that was right.
