Symptomtext
ADR occurred prior to arrival. NKA. PT presented to ED 1/25/21 with nausea, decreased appetite that is documented to have started 2 weeks prior. PT received first Moderna Covid vaccine 1/6/21. Documentation in ED record notes patient tolerated Covid vaccine fine. No fever or chills, no vomiting or diarrhea. Patient was treated with normal saline bolus and discharged home same day with instruction to follow up with PCP. .PT seen by CNP 2/2. Documentation notes PT ?started prior to vaccines and became more severe after vaccine?. Documentation notes ?rash and then symptoms worsen. Took allergy relief".
