Symptomtext

per spouse - client received vaccine at approximately 1300 on 1/13/21. They went home had dinner, no signs of distress noted. Spouse went to bed at 2100, leaving client in den reading the paper. At approximately 2300, spouse awoke thinking client was snoring and he was not. She noticed he was purple and clenching his jaw while making gurgling noises. She called EMS who arrived and worked client for 20 minutes prior to transportation to Hospital. Client was pronounced expired later that evening.

MOD012L20A) for COVID-19 vaccination. No Medical History information was reported. On 14-Jan-2021, the patient received first dose of mRNA-1273 (Moderna COVID-19 Vaccine) (Intramuscular) 1 dosage form. PATIENT STATES SHE TESTED POSITIVE ON NOVEMBER 20, 2020 AND AFTER RECEIVING VACCINATION FELT THE SAME SYMPTOMS SHE EXPERIENCED WHEN SICK. PATIENT STATES IT STARTED APPROXIMATELY 4 HOURS AFTER THE VACCINE AND SHE JUST DIDN'T FEEL WELL. PATIENT STATES SHE EXPERIENCED BODY ACHES, HEAD ACHE, AND EXTREME FATIGUE. PATIENT STATES THIS LASTED UNTIL NIGHT FALL ON THE 3RD DAY AFTER RECEIVING THE VACCINATION.

Symptomtext

On 01/22/2021, small dot started to itch and was red. the redness continued to increase and almost had appearance of a "bullseye". One 01/25/2021, took an oral dose of diphenhydramine and by 01/26/2021 seems to be better.

Symptomtext

Pt advises the area where vaccine was received 2-3 days later site became red/started itching and she also experienced redness down her arm approximately 2-3 from vaccine site. Pt advises she took Benadryl and it helped with the itching but the redness remained. She continues to have some redness but advises it is starting to fade some.
