Symptomtext

This is a 75-year-old male with past medical history of diabetes mellitus, hypertension, Congestive heart failure and thyroid disease. The patient presented to the hospital with complaints generalized weakness, fever and congestion. He states that he was tested positive for COVID 1 week ago and has been feeling fatigue. He had the Johnson and Johson vaccine for COVID. The patient was seen in the ED on 08/07 and 08/08. X-ray of the chest done on admission shows Worsening pneumonia. On examination, patient complains of muscle pain and fatigue. 8/8/2021 tested positive

Symptomtext

-he came to the hospital with a chief complaint of worsening cough and altered mental status has reported by than memory care unit staff. History not obtained from the patient due to advanced dementia. Daughter present at the bedside provided the history. She reported that apart from altered mental status agitation and worsening cough with yellow color expectoration the patient does not have any other symptoms like fevers, chills, abdominal pain, nausea, vomiting, chest pain, shortness of breath, palpitations, burning or painful urination, diarrhea. 01/03/2022 tested positive
