Symptomtext
Pt presented to clinic for influenza vaccine. Pt stated has history of reaction to vaccine for itching and redness/rash. Pt was advised to wait additional 30 mins post administration to watch for any adverse reaction. Approximately 1005 vaccine was administered and at approximately 1020 pt was complaining of itching and rash around neck and face. Pt was assessed and was noted to have a slight rash around the back of neck and left side of neck. Vitals were take at 1021 BP-195/135, HR 106, O2- 95%, RR-16. Pt did not complain of chest pain or headache nor any other symptoms. Pt was alert and oriented with no signs of distress, he expressed history of high blood pressure and that he had not taken his BP Meds today. We continued to monitor pt and reassess vitals q5-10 mins by RN and Technician. Consulted with doc for further instruction, pt still communicating well and no other issues, rash began to go away and itching began to subside. Pt was advised to f/u with PCM to receive further testing for possible flu allergy/consideration for exemption. Pt was also instructed to f/u about elevated BP concerns and to take medications as prescribed by PCM. After continuous monitoring pt stated that he felt better and that he would f/u with PCM for additional screening and that if any symptoms return/worsen to contact EMS. Pt was released back to duty. Manual BP taken @ 1024: 218/140, HR: 95 Temp 98.4, O2-97%, RR-16 1030: BP-191/125, HR-92, O2-95%, RR-14 1038- BP- 163/115, HR 82, O2-96%, RR-16 1045- BP-178/121, HR 83, O2- 95%, RR 16
