Patient reported to the front desk following discharge and labs reporting that she was experiencing shortness of breath post injection of Flucelvax and Heplisav B. Patient was brought back and had visibly swollen and watery eyes. While walking to exam room she states that she has chest pain. Code orange was called at 1050. Dr. evaluating patient. Patient complains of itchy throat and swollen eyes. Benadryl 50 mg ordered and administered by RN to the LD IM. Patient was placed on 4L O2 via NC Patient remains aaox4 and continues to complain of itching throat and pain with speaking. Solumedrol 125 mg ordered and administered IM to the RD by RN. IV 22g started to R AC by RN. IV flushing well. 1104- O2 decreased to 2L NC. 1110- patient reports eyes and throat feel a little better, but she is groggy and feels congested. Oxygen discontinued and patient continues to saturate at 100 %. Patients husband was notified of allergic reaction and advised to pick patient up from clinic. He states that he is on his way. Patient given water and is able to tolerate with no aspiration. Continued to monitor for improvement while awaiting spouse arrival. IV was discontinued at 1138 with catheter intact and no visible redness or inflammation to VI site. Patient was provided with home care instructions by PA-C: Benadryl 50 mg every 6 hours for the next 24 hours. If unable to tolerate 50 mg then it is ok to take 25 mg. Next dose will be at 1700. Per Dr., she would advise that the patient take Zyrtec Non-Drowsy for the next two days. Patient advised that should she experience shortness of breath, difficulty breathing, mouth/lip/tongue swelling, difficulty speaking, chest pain, facial swelling to call 911. Patient verbalized understanding. Written instructions provided as well. Patient left in stable condition with spouse at 1150.
