severe malaise and lethargy for two days; mild shortness of breath and cough productive of clear mucous for 5 weeks with slow improvement; This could be a conincidental illness with symptoms starting after the vaccine but I had no illnesses for many months prior to this. Though symptoms haven't resolved completely, they seem to be improving and I'm reporting in case there are others, especially if they are on Remicade. There was a much milder, but similar response following my fourth COVID vaccine which was also Moderna and I had no issues at all with the prior 3 Pfizer COVID vaccines.

Patient received the annual flu shot and new Spikevax covid vaccine on 10/11/23. Patient called the pharmacy on 10/12/23 and asked to speak to someone about the vaccines she had received. The patient asked the pharmacist if having chest pain and swelling of the lymph nodes the day after getting vaccines was normal. The pharmacist told her we sometimes see flu-like symptoms for a day or two after getting these vaccines but if she is having chest pain we would recommend getting medical attention. Pharmacist enquired if there were any other factors that could have caused this, such as physical exertion, exercise or other illness. Patient said nothing out of the ordinary. Pharmacist reaffirmed getting medical attention. Patient did not seem very concerned and said that was all the info she needed for now.

developed a rash all over my body, red patches every where even on my face; every joint in my body hurt; This spontaneous case was reported by a patient and describes the occurrence of PAIN (every joint in my body hurt) and RASH (developed a rash all over my body, red patches every where even on my face) in a 56-year-old female patient who received mRNA-1273.815 (SPIKEVAX 2023-2024) (batch no. AU3833D) for COVID-19 prophylaxis. No Medical History information was reported. On 31-Oct-2023, the patient received dose of mRNA-1273.815 (SPIKEVAX 2023-2024) (Intramuscular use) .5 milliliter. On 06-Nov-2023, after starting mRNA-1273.815 (SPIKEVAX 2023-2024), the patient experienced RASH (developed a rash all over my body, red patches every where even on my face). In November 2023, the patient experienced PAIN (every joint in my body hurt). At the time of the report, PAIN (every joint in my body hurt) had not resolved and RASH (developed a rash all over my body, red patches every where even on my face) was resolving. For mRNA-1273.815 (SPIKEVAX 2023-2024) (Intramuscular use), the reporter did not provide any causality assessments. Concomitant and treatment medications were not reported. Reported that every joint in body hurt and continues to hurt as of 10-Dec-23 and has not gotten any better.

Pt came to pharmacy today, she said that she got her Monderna Covid-19 vaccine (Spikevax) on 11/01/2023. Upon inquiring and looking at the vaccine site I observed redness, swelling & soreness (Local Reactions at Injection Site). Pt also mentioned that two days later on (11/03/23), she felt a single episode of dizziness (almost vertigo feeling) for a brief moment. She is feeling fine now.

Patient contacted our office 24 hours of receiving a vaccination. Patient was experiencing joint type pain at the shoulder. Denies radiating pain. Per patient the vaccine was given 2.5 finger width below the acromion process. Director suggested patient use NSAID's and Ice to reduce any swelling. Recommended seeing primary care provider if pain increases. Followed up with Patient 48 hours after. Patient stated the pain has decreased, but not subsided. Patient is taking an NSAID and stated it has helped.

The night after administration at 3 am, she had shivering, and freezing. When she got up she had temperature of 101 degrees F. She took aspirin, temperature came down was 99 for a few hours. On Wednesday, 10/11/23 about 4 pm, you had double vision in both eyes, wet eyes, unable to navigate in chair and needed help getting up (could not walk on her own). Sat back down and stayed for a while, and it eased up, and then it did it again about 5:30 pm. Called eye doctor about 6 pm, to let know about double vision (informed doctor had shots on Monday). Was told by eye doctor to watch out for stroke like symptoms and if occurred patient should go to hospital.

Patient states he had radiating pain (radiculitis) and muscle spasms in left shoulder and neck starting the day after the injection and has gotten worse and has been unable to sleep. Patient symptoms got worse 10/10/23 (day 9 after injection). Patient started taking a medrol dose pack starting 10/10/23. Patient went to orthopedic prescriber 10/12/23 and he was given flexeril and meloxicam. Orthopedic prescriber took xrays and diagnosed patient with arthritis in neck. Patient was unsure this was from the vaccine but is highly encouraged to fill out a form.

gave Spikevax to pt on 10/9/23 at around 9:45am. Pt said reaction started about 4 hours later around 2pm with fever of 101F, chills, and rash on both arms and body trunk. Today 10/10/23 she called pharmacy at 4:55pm to informed pharmacy of reaction. She said the rash is now only on her right arm and trunk. She took a benadryl last the night on 10/9/23 for the itch to help her go to sleep.

No adverse event; Patient received his Spikevax 2023 2024 vaccine after Moderna Direct called the Pharmacy and told the patient that the vaccine was a ?bad batch" bad to use.; This spontaneous case was reported by a pharmacist and describes the occurrence of POOR QUALITY PRODUCT ADMINISTERED (Patient received his Spikevax 2023 2024 vaccine after Moderna Direct called the Pharmacy and told the patient that the vaccine was a ?bad batch" bad to use.) and NO ADVERSE EVENT (No adverse event) in an adult male patient who received mRNA-1273.815 (SPIKEVAX 2023-2024) (batch no. AU3833D) for COVID-19 prophylaxis. No Medical History information was reported. On 29-Sep-2023, the patient received dose of mRNA-1273.815 (SPIKEVAX 2023-2024) (unknown route) 1 dosage form. On 29-Sep-2023, after starting mRNA-1273.815 (SPIKEVAX 2023-2024), the patient experienced POOR QUALITY PRODUCT ADMINISTERED (Patient received his Spikevax 2023 2024 vaccine after Moderna Direct called the Pharmacy and told the patient that the vaccine was a ?bad batch" bad to use.). On an unknown date, the patient experienced NO ADVERSE EVENT (No adverse event).
