Patient has received 2 Pfizer COVID vaccine 5-11 year (11/07/2021 and 12/03/2021) , COVID vaccine pfizer bivalent 5-11 year (11/22/2022). Today patient received dose of Moderna 2023-2024 spike protein COVID vaccine 6mo-11year (12/27/2023) patient waited 15 minute per protocol and upon assessment after waiting patient developed a round red half inch circle at the injection site, and located under the adhesive bandage placed, patient had 2 hives on left lower arm region and flushed red cheeks. Patient reported no itching however, scratching her above her left arm (neck) and above left arm, and lower leg. Patient had no reports of breathing concerns, dizziness, lightheaded, diarrhea, nausea vomiting, and constipation. Patient did develop an increase in thirst had 2 glass full of water (20 oz) total approximately. Per provider vitals obtained all WNL. Given Benadryl 33.7 mg at 11:47 am. Patient was doing well and discharged per provider.
