No additional AE reported; PROQUAD was administered following a temperature excursion; This spontaneous report was received from a medical assistant referring to a patient of unknown age and gender. The patient's pertinent medical history, concurrent conditions, drug reactions/allergies and concomitant medications were not provided. On 04-JAN-2022, following a temperature excursion (product storage error), the patient was administered a dose of measles, mumps, rubella and varicella (oka-merck) virus vaccine live (PROQUAD) lot # U002534, expiration date on 24-JUL-2022, for prophylaxis (strength, dose, route of administration and anatomical location was not reported). Vaccine were improperly stored at a temperature of -11?C, during a time frame of 3 hours. Previous temperature excursions were reported at -14?C to -10?C for 8 hours and -10?C to 8?C (-8-8?C) for 1 hour. No additional adverse event (AE) was reported (no adverse event).

Symptomtext

Patient presented to clinic for a well child check, returning to the state (after living out of state). MA gave Kinrix and MMRV per MN MIIC. After both combination vaccinations were given with parental consent, mother provided a vaccination record noting that he was up to date on his DTaP, MMR, and varicella. He was due for his 4th IPV dose. With giving Kinrix and MMRV, he got extra doses of the MMR and varicella. He also got a dose of DTaP that he was not due for and due to his age should have been switched to Tdap. This is a vaccine administration error and no adverse effects were noted. No additional care was needed. Patient's mother was contacted after the visit to notify her what happened.
