Child seen for routine well child check on 7/27/22. He was brought to the ED the following morning 7/28/22 after cardiac arrest.

Cough Crying Prone position Rash erythematous

Symptomtext

Mom and baby (patient) came in for a 2mo WCC. Pt well and healthy was due for 2mo immunizations. Dr. approved and ordered 2mo Vaccinations which are Pediarix, Hib, PCV13, and Rotavirus. I prepared immunizations and went into the room to begin administering them. I had mom hold baby in arms upright and began to administer Rota virus orally. Patient was taking vaccine well but, then began to cough then I instructed mom to turn baby baby prone and lightly tap his back. Patient began to cry. I went for Dr. and she came in to assess the we observed red dots on baby's bilat legs. Mom stated baby was putting his weight on mom. Dr. evaluated patient. He was alert and stable . No more immunizations given at this visit. Patient waited 20mins in clinic and Dr. re evaluated baby again He was alert and stable. Mom sent home with instructed to watch for signs of lethargy, dehydration or any new spots on legs to take baby immediately to ER . Patient will get the rest of 2mo Vaccines at next WCC.

Symptomtext

Patient was seen 3 weeks prior for routine vaccinations which she received. Patient had another visit in which the same routine vaccines were pended, ordered and given again by RN. Inapproporate interval of vaccines given too close together which will require the patient to be revaccinated again at the proper interval. Vaccines- Dtap, Hep B, Polio, Prevnar 13, Hib, Rotavirus

Symptomtext

He received these vaccines at about 09:00 on 1-27-22. He was fine until about 11:55 that day when he went to sleep, awakening only a bit at which times he was fussy and he would reposition himself. Temp was 102. No respiratory difficulties. The next morning temp was 101, mom gave acetaminophen. By 12:30 on 1/28/22 he finally drank 3 ounces of pedialyte, was smiling some and jumping some. Because of poor p.o. intake he was seen at ER on 1-28-22. Respiratory panel was normal, COVID-19 and flu tests were normal. He had a slightly red throat. The assessment was that Noah was having a vaccine reaction. On 1-31-22 mom noted that he had mostly just slept for 48 hours starting at 11:55 on the day of the shots, he also had a 103 temperature on 1/29 at night, he would occasionally take formula but not much, and if alert wanted to be held. At the time of exam on 1-31-22 he was alert and drooling, had normal behavior, no rash, slight rhinorrhea?... It appeared that he was back to normal by the time of the exam. Noah had no difficulty with his previous shots.

Symptomtext

Mother reports that 3 days after patient received the Rotavirus vaccine he had bloody bowel movements. Mother said that first bowel movement that had blood occurred on 12/24 at 2:40 pm. Patient had another bowel movement with blood at 4:30 and then a few minutes later had a stool that was "all blood." Patient was evaluated at Hospital Emergency Department on 12/24 and discharged home after completing stool testing. Stool testing negative for pathogens. Mother reports that patient had about 12 bowel movements total on 12/24. Mother said on 12/25 patient had one bowel movement with blood in it and then they did not see any further blood in stool. Mother said patient did not have a fever or any other symptoms after receiving vaccine. Mother said patient usually has 4-5 bowel movements a day.
