Patient parent called and notified office of seizure 05/25/2022. Went to outside ER for treatment 05/25/2022. Positive for Covid 05/25/2022. Sent home. Told to give Tylenol and Ibuprofen rotating.

Symptomtext

S: Patient returns 10 days following MMR injection with complaints of bruising and sore shoulder. Patient reports that the visible bruising appears to be getting better, but seems to have increased muscle pain especially with movement. No erythema, focal swelling, purulence, fever, chills, malaise. O: Right deltoid with large patch of ecchymosis ranging from blue to yellow. Patient is tender to palpation over deltoid and distal bicep. Full range of motion with discomfort. A: Ecchymosis and possible hematoma Adverse vaccine reaction P: Likely self limited. May treat with RICE, NSAIDs, Muscle use. Return if and new/worsening symptoms.

24 hr post vaccination parent reported visible redness, to bilateral thighs. Parent reported warm to the touch and painful. Pain lasted q 24hr and reduce to only tender feeling 48hr post injection. Right leg redness present from below injection site on anterior lateral thigh extending almost to the hip and down to the knee cap. Left leg redness occurred below the knee extending towards her ankle

Symptomtext

Rash started to appear around varicella injection sight on patient's left thigh on 01/03/2022. It then spread to her trunk, arms and neck the next day and injection sight appears bruised. Followed up with patient's mother on 01/05/2022 and mom states it looks like hives and is also on her face. Directed mom to contact their health provider and get recommendations on if she needs treatment and to see if it is break through chicken pox or a vaccine reaction.

Patient had no reported side effects; was accidently administered to a patient intramuscularly instead of subcutaneously today.; vaccine administered to a 28-years-old patient; This spontaneous report was received from certified medical assistant and refers to a 28-year-old patient of unknown gender. The patient's medical history, concurrent conditions, and concomitant therapies were not reported. On 05-AUG-2022, the patient was administered a dose of measles, mumps, and rubella (wistar ra 27-3) virus vaccine, live (M-M-R II) subcutaneous injection, recombinant human albumin (rHA), 0.5 ml, lot # U006487, expiration date: 26-FEB-2023, administered intramuscularly for prophylaxis (strength, and route of administration were not provided) (incorrect route of product administration, product administered to patient of inappropriate age). Patient had no reported side effects (no adverse event).

not experiencing any additional side effects; Patient was inadvertently administered the MMR-Il vaccine instead of an MMR-Il titer test; This spontaneous report has been received from a health care professional referring to his 29 years old daughter. The patient's pertinent medical history, historical drugs, concurrent conditions, concomitant therapies, and previous drug reactions or allergies were not reported. On 29-NOV-2021, the patient was inadvertently vaccinated with measles, mumps, and rubella (wistar RA 27-3) virus vaccine, live (M-M-R II) for prophylaxis instead of being performed an M-M-R II titer test (vaccination error) (lot number U006487 has been verified to be a valid lot number, the expiration date was reported and established as 26-FEB-2023; dose, vaccination scheme, route of administration and anatomical site of vaccination were not reported). The reporter stated that he believed that the patient was not pregnant, but she was worried since she and her husband were trying to become pregnant. On 07-DEC-2021, the reporter had contacted Merck and informed that was measles, mumps, and rubella (wistar RA 27-3) virus vaccine, live (M-M-R II) was given inadvertently to the patient at new job and it was a medication error. The patient should wait for 28 days after the vaccine was administered. The reporter was requested to inform if the patient becomes pregnant within that time frame for follow up. The patient was not experiencing any additional side effects (no adverse event). No further information was available.
