No additional AE reported; Medical Assistant reporting improperly stored VARIVAX was administered to a patient; This spontaneous report was received from a certified medical assistant referring to a 12-month-old patient of unknown gender. No information was provided regarding the patient's medical history, concurrent conditions, drug reactions or allergies and concomitant medication. On 25-AUG-2022, the patient was vaccinated with an improperly stored dose of varicella virus vaccine live (Oka/Merck)(VARIVAX) (1 dose) lot number U007475 and expiration date of 03-AUG-2022 for prophylaxis (dose, route of administration, anatomical location, lot number and expiration date were not reported). The vaccine was exposed to a temperature excursion of -10?C up to 8?C (-9.6C) for 1 hour and 0 minutes and 0 seconds, there was no previous temperature excursion. No additional adverse event was reported.

Patient received ProQuad and Varicella at this visit; This case was retrieved during the Vaccines Adverse Event Reporting System (VAERS) monitoring on 12-JAN-2022 with the following reference number: 1745229-1. The original narrative from the sender is as following: "Patient received ProQuad and Varicella at this visit." The causal relationship between Vaxelis and the event "Patient received ProQuad and Varicella at this visit" was assessed as not applicable due to the nature of the event. Of note, the event do not concern Vaxelis, but rather ProQuad and Varicella vaccines. Vaxelis should had been considered as concomitant. However, the case was databased to mirror what was reported in VAERS. No further information is expected

Inappropriate schedule of product administration No adverse event

Symptomtext

Client had lost his childhood vaccine record and needed to start from scratch. He needed multiple vaccines and was agreeable to that plan of care. He was given Varicella on 9/3/21, which was only 21 days from another live vaccine (MMR) on 8/16/21. These vaccines need to be given on same day or 28 days apart. The nurse knew she had done this immediately and brought to my attention. Client without adverse reactions. Will make client aware that vaccine will need to be repeated.
