Per Dr.: Left thigh area of erythema was outlined with a pen. There is a mild increase in heat and tenderness but no localized blanching or reaction. 12 x 14 cm area. It became red and painful. He is still walking normally.

On 06-SEP-2022, the patient was vaccinated with an expired dose of measles, mumps, rubella and varicella (oka-merck) virus vaccine live (PROQUAD) recombinant Human Albumin (rHA), 1 dose, lot #U007505, expiration date: 04-SEP-2022 (dosage regimen, anatomical location and route of administration were not provided) administered as prophylaxis (expired product administered).
