Malaise Syncope

Symptomtext

Patient received HPV, hepatitis A, and hepatitis B vaccines. Was educated about dizziness or fainting feeling potentially after HPV vaccine. Patient walked to checkout desk with mom. Patient stated to mom that she was not feeling well and fainted. Desk staff alerted nursing who alerted the providers as well. Patient did not hit head. Was positioned to her back and legs lifted. Eventually moved to room where vital signs were taken and she was given apple juice and graham crackers. Physician evaluated patient and found patient was safe to go home. No injuries found.

Symptomtext

A couple hours after administration of vaccine patient started experiencing lightheadedness and took a shower then felt like she was going to pass out. She also complained of chills in ER. She was seen by PCP the next day and she reported body aches, hot and cold sweats and head pain. Was given dose of Toradol in the office for pain symptoms.

dizziness and headache and extreme fatigue "the most tired I have ever felt in my life." He described vertigo. Then yesterday, he struggled to wake up in the morning, was very tired and came downstairs crying around 10:30, he felt like he was still extremely tired. Then became confused, like he was seeing things that weren't there -eg bananas looked like a dog

Symptomtext

No additional adverse events; Medical assistant calling in a temperature excursion; Information has been received from a medical assistant, referring to a 16 year old patient of unknown gender. The patient's medical history, concurrent conditions and concomitant therapies were not reported. On 01-OCT-2021, the patient was vaccinated with an improperly stored dose of hpv rl1 6 11 16 18 31 33 45 52 58 vlp vaccine (yeast)(GARDASIL 9) lot # U008048, expiration date on 01-OCT-2021 (anatomical location and administration route were not reported)for prophylaxis. No additional adverse effects were reported. The vaccine had a temperature excursions of 78 Fahrenheit (F) during a time frame of 49 hours. It was confirmed that no previous temperature excursion occurred.
