Patient received the Men B and HPV Vaccine, few minutes after HPV patient was receiving TB test when the MA notice her head tilting to the left side and eyes rolling with mild shaking lasting for 30seconds. Provider came in Patient was on the bed and she was laid down with feet elevated and MA recheck Vitals and kept in the room for 20min. patient was sit up position and gave some cold water as provider indicated. Patient Blood pressure was low and patient had Dizziness 95/60 Provider indicated to send patient to ER. Called patient mother at 4:45pm and followed up patient was discharge she was doing beter.

21 year old female with no significant past medical history who presents with 4 week history of progressively worsening vision, muscle spasms and paresthesias. Pt. shares the above symptoms started immediately after receiving her first HPV shot in her right arm. Symptoms of progressive vision loss worsened to the point where she now has to wear contact lenses. She reports since receiving her contact lenses she notices continued worsening of vision, reports when the light are dim it is very hard for her to see. She endorses bilateral floaters as well. She reports an aching of bilateral eyes as if she has been staring at the computer screen for prolonged periods of time. She shares bilateral tinnitus with muffled hearing bilaterally. She shares her muscle spasms that started 2 days after receiving the HPV shot started in her lower extremities and progressed upward to her arms. She denies muscle weakness or atrophy. They are associated with paresthesias that are worse in the evening and prompt patient to move her legs. Symptoms are improved with movement. She reports 1 week history of palpitations after immunization as well that have since resolved. Durations 10 minutes occurring hourly. She denies headaches, weight loss, fever, chills, shortness of breath, cough, chest pain, recent travel. She has not been sexually active for 2 months LMP was 2 weeks ago. She recently stopped Aubra, OCP, 1 week after her vaccination. Social: non-contributory FHx: Brother wears glasses, Uncle: Thyroid dx.

patient felt dizzy immediately after receiving these 2 vaccines, she did not loose consciousness and was able to talk. patient was asked to lay down on exam table, rechecked her breathing, heart rate, skin and lips/ tongue and oropharyngeal cavity which were all normal. patient was monitored for 15 minutes, given water to drink. patient was back to her baseline within 10-15 minutes and was able to go back home walking with her mother.

Patient reported the following on a call of 03/07/2022, that she presented hives on the whole body accompanied with itichiness form the neck down, for four days since - 02/24/22 to 02/28/22-, also report hives accompnied with itchiness on the injection site, did not reported other symptoms as trouble to breathing, tireness or dizziness or swelling, did not took any medication, but use regular body lotion to help to calm the itchiness, neither consulted to the doctor or went to the ER. On the call of 03/07/2022 reported feeling good all and the symptoms diseapeared by then.

Patient inadvertently received MenB vaccine. Patient was not due for Men B, and just due for Meningococcal Conjugate. Men B was pulled from refrigerator and administered to patient. Patient received planned vaccines of Tdap, Meningococcal Conjugate and HPV. Patient was notified in person and mother was notified via phone. Updated system , VIS sheets, and a copy of signed vaccine consent sent home with patient. Patient experienced no symptoms and returned to class 15 minutes after vaccine administration. Snack and water eaten by student while waiting and ice pack applied to arms . The patient received a third dose of HPV vaccine. The patient had initiated the series before age 15, which warranted 2 doses. A letter was sent home to the patient along with consent form for the vaccines needed. The consent form had all 4 vaccines (Men B, Influenza, HPV, and Covid) highlighted as due. After administration, the mistake was caught that the patient did not need a 3rd dose of HPV due to age of series start. The clinical supervisor was notified and a meeting was held on 2/14/2022 to discuss actions to prevent this from happening again. The formed plan included printing off a list the day of the appointment for each patient in landscape form. Reviewing the database, administering vaccine, than writing on the database which vaccines were given and the list was sent home with the patient.
