Symptomtext

A Medical Assistant administered 9 doses of IPV in one injection. Poison Control, Provider, and Medical Director provider notified. Poison Control states patient should be fine, just watch for abnormal reactions. They state nothing else can be done. Provider assessed the injection site when parent noticed some swelling and lump form around the injection site. There was no other known symptoms around administration. Patient came in for a follow up visit on 9/2/22. Provider evaluated patient's injection sites in his legs. His left leg where the IPV was given did not show any type of reaction. But his right leg was present with a basball sized area of erythema with some swelling, tender with touch, and came into clinic Limping on the right leg. Patient does not have any other symptoms. Patient's symptoms from his reaction is improving. Provider advised to continue watching for any abnormal symptoms, use ice packs, and Ibu for pain. If this does not resolve or worsens, patient needs to come back in for evaluation. The Medical Assistant who administered the vaccines on that day has been discharged from the clinic.

No additional AE reported.; expired PROQUAD administered to patient.; This spontaneous report was received from a Medical Assistant, concerning a patient of unknown age and gender. Information regarding the patient's medical history, concurrent conditions, drug reactions or allergies was not provided. Concomitant therapies included sterile diluent (manufacturer unknown). On 10-OCT-2022, the patient was vaccinated with an expired dose of measles, mumps, rubella and varicella (oka-merck) virus vaccine live recombinant human albumin (rHA) (PROQUAD) subcutaneous injection, at a dose of 0.5 milliliter, lot #U010743, expiration date: 03-OCT-2022, administered for prophylaxis (route of administration and anatomical location were not provided) (product storage error). The reporter stated that the product did not experience any temperature excursions through date of administration. No additional adverse event was reported.

No additional adverse event; One dose of Proquad was administered after a temperature excursion; This spontaneous report was received from a medical assistant, and refers to a patient of unknown age and gender. The information about patient's concurrent condition, medical and drug history and concomitant medications was unknown. On 21-NOV-2021, the patient was vaccinated with one dose of measles, mumps, rubella and varicella (oka-merck) virus vaccine live(PROQUAD) lot # U010743 and expiration date: 03-OCT-2022, dose: 0.5 milliliter (route of administration and anatomical location were not provided) as prophylaxis. On an unspecified date in November 2021, the administered doses of measles, mumps, rubella and varicella (oka-merck) virus vaccine live(PROQUAD) had a temperature excursion (product storage error) as follows: -14?C to -10?C lasted for 45 minutes and -10?C to 8?C (-9.92?C) lasted for 15 minutes. No previous temperature excursion and no additional adverse event reported.
