Symptomtext

On June 22, 2022, pt. arrived around 9:00AM with mother and 2 siblings for immunization visit. Pt did not receive any immunizations until age 12 due to mother's beliefs against vaccines. Mother reports they have moved and he needs all his vaccines to enroll in school. Mother desires pt. to receive all vaccines that he is due for. Nurse attempted to reduce cost to patient by suggesting combination vaccine MMRV proquad without checking age range for vaccine. Patient was checked in around 9:30 AM and Nurse began preparing vaccines for both children. After counseling, mother agreed for vaccines and signed for them around 9:45AM. Vaccination of pt. began around 9:50AM and MMRV proquad was given in right arm subcutaneous. Nurse realized error at 10:00AM and patient stayed in office until 10:15AM. Pt denies SOB, swelling of throat or tongue. Pt reports only heavy, sore arm feeling due to multiple vaccines.

Symptomtext

Within 15 minutes reaction occurred: Both injection sites had a 1 inch circular surrounding erythema. The left thigh (MMRV) had a 0.5 cm wheal (hive) present just above the injection site. The right thigh (DtaP) had approx 4-5 0.1 cm papules surrounding the injection site. The child was given 2.5 ml (6.25mg) Children's Benadryl and within thirty minutes, the wheal (hive) and papules hade resolved. The surrounding erythema had improved. He was sent home. Approx. thirty minutes later mom called stating he was vomiting. She was instructed to take the child to the ER. I spoke with her today and she did not take him to the ER. She states he projectile vomited four times and has since been his normal self. She denies him to have fever, rash or other symptoms/reactions.

Symptomtext

Pinpoint red papule noted on lateral Left thigh laying over a swollen red and warm upper thigh including the Left knee joint as well. Associated with limp and pain. Oral dosage of motrin given the next morning by mother was non-responsive. After two 1/2 days or oral steroids given at 1mg /kg bid with 100 % resolution.

Date of administration was Mar 23, 2022.; This spontaneous report was received from a nurse and refers to a patient of unknown age and gender. No information regarding the patient's concurrent conditions, medical history and concomitant medications was provided. On 23-MAR-2022, the patient was vaccinated with improperly stored (product storage error) dose of measles, mumps, rubella and varicella (oka-merck) virus vaccine live (recombinant Human albumin) (PROQUAD) lot #: U010745, expiration date: 03-OCT-2022 (exact dose, strength, site and route of administration were not provided) for prophylaxis. Temperature excursion information: -8.3 degree Celsius for 1 hour 1 minute. No previous temperature excursion. There was no additional adverse event reported.

PROQUAD has been administered to multiple patients since the temperature excursion; This spontaneous report was received from a medical assistant referring to a 4-year-old male patient. No information was provided regarding the patient's historical conditions, previous drug reactions, allergies, concurrent conditions or concomitant medications On 20-JAN-2022, measles, mumps, rubella and varicella (oka-merck) virus vaccine live (recombinant Human albumin) (PROQUAD) was involved in a temperature excursion and on that day, the patient was vaccinated with this improperly stored vaccine once at left thigh for prophylaxis (lot # U010745 with an expiration date on 03-OCT-2022; dose and route of administration, were not reported). The freezer had a lot of ice, while it was being cleaned, the door was open for 15 minutes, and there was temperature excursion. The dose of measles, mumps, rubella and varicella (oka-merck) virus vaccine live (recombinant Human albumin) (PROQUAD) was exposed to a temperature excursion of 21.1 degrees Fahrenheit (?F) during a time frame of 2 hours 34 minutes. The temperature excursion occurred as a result of a reading from a Digital Data Logger. There was no prior temperature excursion and no adverse event was reported. This is one of two reports received from the same reporter.

The patient's pertinent medical history and drug reactions/ allergies were unknown. Patient was not being administered any concomitant medications at event time. On 08-OCT-2021, the patient was vaccinated with an improperly stored measles, mumps, rubella and varicella (oka-merck) virus vaccine live (albumin status of rHA (recombinant human albumin)) (PROQUAD) (lot # U010745, expiration date 03-OCT-2022; strength, dose and route were unknown) for prophylaxis (product storage error). Previous temperature excursion was minus 10 degree Celsius to 8 degree Celsius (0.91 degree Celsius) for 6 hours on 04-OCT-2021 and minus 10 degree Celsius to 8 degree Celsius (1.45 degree Celsius) for 6 hours. Temperature excursion of this vaccine on 08-OCT-2021 was minus 14.6 degree Celsius for 6 hours. No additional adverse event was reported. Additional Excursions had occurred after patient was administered the improperly stored measles, mumps, rubella and varicella (oka-merck) virus vaccine live (PROQUAD): 13-OCT-2021 from minus 10 degrees Celsius to 8 degree Celsius (0.54 degree Celsius) for 13 hours. No additional adverse event; Improperly storage vaccine was administered; This spontaneous report was received from a healthcare business professional referring to a patient of unknown age and gender. The patient's concurrent conditions, medical history and concomitant therapies were not provided. On 08-OCT-2021, the patient was vaccinated with measles, mumps, rubella and varicella (oka-merck) virus vaccine live (albumin status of recombinant human albumin [rHA]) (PROQUAD) (strength, dose, route, frequency, were not provided) for prophylaxis; lot No. U010745 has been verified to be a valid lot number for measles, mumps, rubella and varicella (oka-merck) virus vaccine live; expiration date reported as 03-OCT-2022 and upon internal validation established as the same. The reporter stated that the administered dose of the vaccine was improperly stored (incorrect product storage). The administered dose of the vaccine experienced a temperature excursion of -14.6 degrees Celsius (?C), for 6 hours (temperature Excursion had occurred on 08-OCT-2021). It was reported that previous temperature excursion occurred on 04-OCT-2021, from -10?C to 8?C (0.91 ?C) for 6 hours; and on 05-OCT-2021, from -10?C to 8?C (1.45 ?C) for 6 hours. Additional excursion had occurred after patient was administered the improperly stored measles, mumps, rubella and varicella (oka-merck) virus vaccine live (PROQUAD) on 13-OCT-2021, from -10?C to 8?C (0.54?C) for 13 hours. There was no additional adverse event (AE). No product quality complaint (PQC) involved.
