No additional AEs; PROQUAD and VARIVAX administered to 12 month old at same visit; This spontaneous report was received from a registered nurse and refers to a 12-month-old patient of unknown gender. The patient's pertinent medical history, concomitant therapies, drug reactions and allergies were not reported. On 17-JUN-2022, at the same visit, the patient was administered measles, mumps, rubella and varicella (oka-merck) virus vaccine live, recombinant Human albumin (rHA) (PROQUAD), 0.5 ml, lot #U010746, expiration date was not provided but upon internal validation was determined to be 03-OCT-2022; and varicella virus vaccine live (oka/merck) (VARIVAX), 0.5 ml, lot #U010400, expiration date was not provided but upon internal validation was determined to be 01-APR-2023, both vaccines administered subcutaneously for prophylaxis (vaccine overdose). No additional adverse events (AEs) were reported.

Case came in with parent for 1 year shots:hepatitis A,MMR,and varicella.Nurse thought she had dose of MMR ll but it was MMR and varicella (Proquad).Proquad dose given sub-q in right thigh.Varivax dose also given in left thigh by sub q route.

No additional adverse event reported; The patient was vaccinated with improperly stored PROQUAD; This spontaneous report was received from a nurse practitioner referring to a patient of unknown age and gender. The patient's concurrent conditions, pertinent medical history, concomitant medications, drug reactions or allergies were no reported. On an unspecified date, the patient was vaccinated with improperly stored Measles, Mumps, Rubella and Varicella (Oka-Merck) Virus Vaccine Live (PROQUAD) rHA(Lot# U010746 and expiration date 03-OCT-2022) (exact dose and frequency were not reported) as prophylaxis. (product storage error). On 09-DEC-2021, the vaccine underwent a temperature excursion of minus 14 degree Celsius to minus 10 degree Celsius for 0 hours 10 minutes and 0 seconds and minus 10 degree Celsius to 8 degree Celsius (7.4 Celsius) for 8 hours 20 minutes and 0 seconds. There was no previous temperature excursion. No additional adverse event reported, no further information provided.
