Symptomtext
Patient was due for a Pneumovax vaccine, order was placed and vaccine given by RN on 11/6/21. Vaccination documentation was not reviewed or discussed with other RN in clinic, another dose of Pneumovax was given to patient on 11/16/21. This near miss was noticed during monthly documentation checks. MD notified. Patient not symptomatic with vaccine.
