No additional AEs were reported, no further information provided.; This spontaneous report has been received from a medical assistant concerning a patient of unknown gender and age. The patient's concomitant therapies, pertinent medical history, and drug reactions/allergies were not reported. On 07-JUL-2022, the patient received improperly stored Haemophilus B conjugate vaccine (Meningococcal protein conjugate) (LIQUID PEDVAXHIB) intramuscular injection, 1 dose (lot number U014483, expiration date reported as 15-SEP-2023; strength and route of administration were not provided) for prophylaxis (product storage error). The temperature excursion was of 32.9 Fahrenheit for 44 minutes. It was reported that there was no previous temperature exclusion. No additional adverse event was reported.

Their pertinent medical history, concurrent conditions, concomitant medications and drug reactions or allergies were not reported. On 05-APR-2022, the patient received a dose of Haemophilus b Conjugate Vaccine (Meningococcal Protein Conjugate) (LIQUID PEDVAXHIB) 0.5 milliliter (ml), administered as prophylaxis (dose regimen and route of administration was not reported). Lot# U014483 has been verified to be valid Haemophilus b Conjugate Vaccine (Meningococcal Protein Conjugate) (LIQUID PEDVAXHIB), expiration date was provided and verified to be on 15-SEP-2023. The nurse reported that there were multiple temperature excursions for Haemophilus b Conjugate Vaccine (Meningococcal Protein Conjugate) (LIQUID PEDVAXHIB), and the vaccine had been administered to the patient (product storage error). A temperature excursion evaluation had not been performed at the time of the call.

No additional AE or PQC reported.; Caller stated PEDVAX HIB was administered after a temperature excursion; This spontaneous report was received from a licensed practical nurse (L.P.N.), referring to a patient of unknown age and gender. Information regarding the patient's pertinent medical history, concomitant medications and drug reactions or allergies was not provided. On 18-MAR-2022, the patient was administered with an improperly stored haemophilus b conjugate vaccine (meningococcal protein conjugate) (LIQUID PEDVAXHIB), dose 0.5 mL, lot number U014483, expiration date was not provided but upon internal validation established as 15-SEP-2023 (route of administration and anatomical location were not reported) for prophylaxis (product storage error). No additional adverse event (AE) or product quality complaint (PQC) reported (no adverse event). No additional information was provided. There were reported temperature excursions of the administered vaccine of 35.1 degrees Fahrenheit (F) for 4 hours 40 minutes, 35.2 F for 54 hours and 35.3 F for 30 minutes. There were no previous temperature excursions. It was not provided if digital data logger was involved.

Symptomtext

Five year old male patient received Hib Vaccine outside of Protocol recommendations. Contributing factors: 1. The child was starting over with vaccines at health department visit with no proof of prior vaccinations. 2. The nurse who ordered the vaccines was not the nurse who performed the well child exam. 3. Staff did not utilize the protocol manual to verify the age limit for administering a Hib vaccine. Via in house interpreter spoke with Father, by phone and he denies the child having any adverse reaction to vaccines.
