Symptomtext

Pt vaccinated at routine wellness exam 11/4/21. Presented with fever later that day. Pt had episode of eyes rolling back and seizure like activity and proceeded to local emergency department for evaluation following this.

Injection site swelling Irritability Rash

Symptomtext

Later that day on the 4th, R leg started to swell at injection site and by the 7th it was 4cm and sticking up 0.5-1cm in height. On the 13th the swelling in leg was down to the size of a mosquito bite. Then on the 11th she developed a rash on L thigh measuring 1.5cm. Mom states she is always cranky and needy after immunizations, for about 5 days. This is the first time that she has had reactions at the injection sites.

Symptomtext

Patient came to clinic for well visit and catch up vaccines as he was behind. He was to get three shots. Kenrix was chosen because it combined two of three vaccines needed which would result in two shots rather than three. Kenrix and Hib vaccines were given at the same time. Patient tolerated the shots well. Mom was given VIS and encouraged to treat with tylenol as needed, and left clinic. It was then discovered that the Kenrix vaccine is for 4 - 6 year olds. Three seperate attempts were made to contact mom to assess the patient's condition without success. A message was left for her to call back but did not get a return call. PA was also notified. Clinic was contacted with a return email stating that if the time intervals parameters were met, the doses could be considered valid.

Patient received routine vaccines as above. Mom noted a few days later a small subcutaneous nodule in right thigh at administration site that has persisted at her 4 month visit
