Several hours post vaccine patient reporting light dry cough, runny nose, shortness of breath at rest. Took 50 mg PO diphenhydramine and symptoms resolved within 45 minutes.

Symptomtext

Mom states pt. started itching her left leg on 4/12/22. She said it was swollen about twice it normally is. No fever. She said her left leg is red and you can see where the vaccines were given. She hasn't complained of it at all. She is walking on her leg normally. Advised mom to give her Benadryl and see if that helps. Spoke to mom on 4/13/22 at 1015am. She states her leg "looks worse" and is really red. It is not purple looking. Pt. still has not complained of it hurting. Scheduled appt for patient to be seen today in office at 130om. Patient was seen in office 4/13/22 at 130pm. See providers recommendations below.

Symptomtext

Patient was administered an expired vaccine. He experienced no side effects or problems. Pharmacist contacted him by phone to inform him of the event and reached out to Merck for guidance. Merck provided additional stability data that supported the expired lot as still being active/safe as long as storage conditions had been maintained. Pharmacist confirmed that storage of vaccine has been maintained properly. Given the information provided by Merck the pharmacist determined the patient did not need to be revaccinated.

On 09-FEB-2023, the patient was vaccinated with a dose of Measles, Mumps, and Rubella (Wistar RA 27-3) Virus Vaccine, Live recombinant human albumin (rHA) (M-M-R II) 0.5 ml once, administered as prophylaxis, lot number U014655 with expiration date 07-MAY-2023. The patient was vaccinated intramuscularly instead of subcutaneous route (Incorrect route of product administration). No additional adverse events were reported (No adverse event).
