Symptomtext

Patient is accompanied by father and sister for vaccination, requesting four vaccines to be done at one sitting for school requirement. history of vasovagal reaction. patient was seated at the bed during vaccination with father sitting next to her. Patient was stable post vaccination. About 4-5minutes after vaccination, patient fainted and hit her face on the fall. No loss of consciousness after the fall. She had chipped her tooth from right front tooth (central incisor) with small laceration on the lower inner and outer lip. The piece of chipped tooth was intact and out of mouth. Tooth root was not exposed. Bleeding of laceration was controlled. No other injury was noted. Patient was laid flat on bed, with direct supervision and frequent vital sign check. BP96-60 HR6; BP94-60 HR72; BP96-68 HR68. Heart rate and rhythm regular. RR 16. O2sat 99-100% on room air. Bilateral radial pulses palpable. Neuro check was normal. wound was cleaned with NS and gauze. Patient was able to gargle and spit to rinse her mouth. No other oral injuries, face/head injury, joint injury noted. She was able to have a small cup of water. Father declined EMS and ED transport for further evaluation and injury. Patient was in the clinic for about 90 minutes with my direct supervision with father in the room. Patient's father connected with his friend at Fire Department and took the patient there to be evaluated. Follow up - I attempted to call for follow up on 7/27/22 at 6:24pm and 6:45 pm, 7/28/22 at 2:23pm, and 7/30/22 at 9:15am - unable to reach father/patient, unable to leave a voicemail due to voicemail box is full.

UPON WITHDRAWAL OF THE INJECTION NEEDLE FROM THE LEFT DELTOID AREA, PATIENT DEVELOPED VASOVAGAL SYNCOPY. PATIENT WAS PLACED IN SUPINE POSITION WITH HIS LOWER EXTREMITY SLIGHTLY ELEVATED. PATIENT RECOVERED WITHOUT LOOSING CONCIOUSNESS. BP 123/83, HR 55. PATIENT VERBALIZED THAT HE DID NOT HAVE BREAKFAST BEFORE VACCINATION. PROVIDED WATER AND ORGANIC APPLE SAUCE. PATIENT LEFT CLINIC FEELING WELL.

Pustule Pyrexia Rash

Symptomtext

2 different type rashes developed about 5 days after getting vaccines first reaction was a rash that started on the cheeks and slowly encapsulated the entire body down to feet was a red rash and lasted for about a week. Then 3 days later developed another rash that was raised pustule like rash noted and had a low grade fever of 100 the entire time.

no adverse event; a dose of VARlV AX was administered intramuscularly to the patient; This spontaneous report has been received from a nurse referring to a 3-year-old female patient. The patient's medical history, concurrent condition and concomitant therapy were not provided. On 24-MAR-2022, the patient was vaccinated with a dose of varicella virus vaccine live (oka/merck)(VARIVAX) 0.5 milliliter, intramuscularly (strength was not reported) (lot number U015677 has been verified to be a valid lot number, expiration date: 18-MAY-2023) for prophylaxis (Incorrect route of product administration). No additional adverse event reported.

No additional AEs were reported; a patient received improperly stored vaccine; This spontaneous report was received from a medical assistant referring to a 13-month-old patient of unknown gender. Information about concomitant medication, pertinent medical history and drug reactions/allergies was not provided. On 09-DEC-2021, the patient was vaccinated with improperly stored varicella virus vaccine live (oka/merck) (VARIVAX) 1 dose (lot # U015677, expiration date 18-MAY-2023; strength, route and anatomical location were not provided) for prophylaxis (product storage error). The administered dose of the vaccine experienced a temperature excursion in 39.55 Fahrenheit degrees (F) for 10 hours. There was no previous temperature excursion. No additional adverse events (AEs) were reported.

The dose of varicella virus vaccine live (oka/merck)(VARIVAX) was exposed to a temperature excursion of 39.55 degrees Fahrenheit (?F) during a time frame of 10 hours. There was no prior temperature excursions. No additional adverse events were reported.
