on 9-19-22, pt noticed one bump on R. arm , now has 5 total bump close to injection site. the injection site has a lump under skin/ pt. The bumps are drying up now . Injection site is sore and itchy.

The patient's medical history, historical drugs, known allergies, concurrent conditions and concomitant therapies were not reported. On 04-AUG-2022, the patient was vaccinated with a dose of varicella virus vaccine live (Oka/Merck) (VARIVAX) lot # U015847 with expiration date of 19-MAY-2023, which had been verified to be a valid lot # for varicella virus vaccine live (Oka/Merck) (VARIVAX), administered once for prophylaxis (strength, dose, and route of administration were not provided) that experienced a temperature excursion of 8.42 Fahrenheit grades (F) for a time frame of 1 hour, 2 minutes. No previous temperature excursion reported, if a data logger was involved was not reported (product storage error). Symptomtext

Both MMR and Varicella given intramuscular instead of correctly subcutaneously. Error noted following charting the immunizations. No immediate local reaction noted on either arm. Completed internal incident report, VAERS report, and spoke with supervisor. Per current guidance, revaccination of MMR and Varicella not necessary and doses given IM count. No other follow up completed at this time.

Symptomtext

This spontaneous report was received from medical assistant referring to unspecified patients of unknown age and gender. The information about patients' concurrent conditions, medical history, concomitant medications was unknown. On 22-OCT-2021, doses of measles, mumps, and rubella (wistar ra 27-3) virus vaccine, live (M-M-R II) and varicella virus vaccine live (oka/merck) (VARIVAX) were exposed to 54.9 Fahrenheit for 2 hours 1 minute. It was reported that there was no previous temperature excursion. On an unknown date in 2021, three improperly stored doses of measles, mumps, and rubella (wistar ra 27-3) virus vaccine, live (M-M-R II) (lot number T012807 has been verified to be a valid lot number, expiration date was 14-APR-2022) and two improperly stored doses of varicella virus vaccine live (oka/merck) (VARIVAX) (lot number U015847 has been verified to be a valid lot number, expiration date was 19-MAY-2023) were administered to unspecified amount of patients for prophylaxis (strengths, doses, vaccinate schemes, route of administration, and anatomical location were not reported) (product storage error). No further information, adverse event or product quality complaints were reported. This is a non- valid report due to lac of patients.
