Symptomtext

Patient received vaccines on 10/26/21. Patient was seen in the clinic on 11/09/2021 for vomiting, decreased urinary output, and lethargy. Mother was instucted to take child to Hospital emergency room immediately after leaving clinic. Child was treated at Hospital and then transferred to another Hospital where he later died.

Patient had fever starting at night on 10/15/21, parent brought patient to ED on 10/17/21 at which time patient had 105 degree F fever and had a 20 second seizure and then vomited, ibuprofen and acetaminophen was given at ED

Symptomtext

Patient presented to clinic on 3/3/22. She had developed a red rash on her trunk and abdomen, on exam was consistent with petechiae. She also had one nosebleed, vaginal bleeding in diaper during the week, and developed a bleeding laceration on her upper and lower lip after falling. She also had right forehead bruising. Patient sent to the emergency room.

On 3/3/22, patient admitted to hospital. Initial platelets were 23 but dropped to 5 the next day, Hb was 9.7 and ANC was 1.25. Patient treated by the hematology team, and discharged after platelets began to uptrend after IVIG treatment. Of note, per the hematology team, CMV IgM was also positive on 3/4/22.

No illnesses or symptoms, however seen on 1/20/22 for COVID19 testing due to positive exposure, was COVID19 negative.

Erythema Urticaria

Symptomtext

Patient experienced redness and hives 15 min. after receiving injection. Denies any swelling of lips or tongue. Patients vitals were taken BP 110/60, Temp 97.3, O2 98%, P 70 Patient's sister gave her Benadryl 25mg. p.o. 15 min later patient returned to class. No hives or redness was present.

no adverse event; Patient was inadvertently and concomitantly administered PROQUAD and VARIVAX on 09/28/2022.; This spontaneous report was received from a healthcare business professional referring to a patient of unknown age and unknown gender. The patient's concurrent conditions, medical histories, concomitant therapies and drug reactions or allergies were not reported. On 28-SEP-2022, the patient was inadvertently and concomitantly administered with measles, mumps, rubella and varicella (oka-merck) virus vaccine live (PROQUAD) orsubcutaneous injection, lot #U022729, expiration date: 16-JAN-2023, therapy type: recombinant human albumin (rHA); and varicella virus vaccine live (oka/merck) (VARIVAX) subcutaneous injection, lot #U016062, expiration date: 20-MAY-2023. both administered for prophylaxis (strength, dose, route of administration and anatomical site of administration were not provided regarding both of vaccines) (overdose). No additional adverse event reported.

Exposure during pregnancy

Symptomtext

Individual was known to be 29 weeks + 3 days pregnant at time of vaccination and should, therefore, not have received varicella vaccine. The screening question "Are you pregnant or is there a chance you could become pregnant during the next month" was appropriately checked yes. The vaccine consent form indicates that the individual received varicella vaccine as this line item is initialed and the administration site was circled as right left arm. When discussion this incident with the vaccination team, the team reports that this was a documentation error, and the individual did not receive the varicella vaccine. This individual was vaccinated during a mass vaccination effort (>400 individuals vaccinated over a two-day period of time and > 1600 vaccine administered), meaning that it's difficult to truly know if the vaccine was not given as the medical team verbally reported.
