Symptomtext

One hour after shot red splotchy rash six inches around injection site. By 5pm, matching rash on right leg. By 6pm rash visible on wrists. By 7pm legs covered in rash, upper arms and neck. Monitored all night for breathing okay. The next morning rash spread up entire trunk and around front. Subsided by 10am follow up appointment but still on every area of body during exam. Anti-histimine prescribed. A single 2.5 dose of zyrtex given at noon. Rash still visible on legs, arms and diaper area at bedtime but largely cleared. Faintly visible at follow up next morning at 10am.

On 25-MAY-2023, the patient was vaccinated with an expired dose of varicella virus vaccine live (oka/merck) (VARIVAX) (dose was 0.5 milliliter (ml), lot number was reported as U016063, expiration date was reported as 20-MAY-2023, route and anatomical location were not provided) for prophylaxis (expired product administered), 5 days past expiration date of 20-MAY-2023. No additional adverse event (AE) reported (no adverse event).

Medical Assistant reporting administration of improperly stored VARIVAX; No additional adverse events reported; This spontaneous report was received from a medical assistant and refers to an unspecified patient of unknown age and gender. The patient's medical history and concurrent conditions were not reported. Concomitant therapies were not reported. On an unknown date, the patient was vaccinated with an improperly stored dose of varicella virus vaccine live (oka/merck) (VARIVAX) (lot# U025791 with expiration date on 19-AUG-2023; lot# U020239 with expiration date on 28-JUN-2023; lot# U016063 with expiration date on 20-MAY-2023) subcutaneous injection, administered for prophylaxis (strength, lot #, expiry date, and route of administration were not provided) which experienced a temperature excursion of 5.4?C Celsius degrees during a time frame of 2 hours 42 minutes. No previous temperature excursions were reported.

No additional AE; VARIVAX underwent a temperature excursion, and the VARIVAX was administered to a patient; This spontaneous report was received from a medical assistant referring to a patient of an unknown age and gender. The patient's concurrent conditions, medical history and drug reactions/allergies were not provided. Concomitant medication included sterile diluent. On 01-MAR-2022 (also reported as 01-MAR-2023), the patient was vaccinated with improperly stored varicella virus vaccine live (oka/merck) (VARIVAX) lot# U016063, expiration date 20-MAY-2023, at a dose 0.5 milliliter (route of administration and dose number were not provided) for prophylaxis (product storage error). The vaccine was diluted with sterile diluent (0.5ml, lot# reported as T029529, expiration date reported as 16-APR-2023). The administered dose of varicella virus vaccine live (oka/merck) (VARIVAX) underwent a temperature excursion of -14.3 Celsius degree for 40 minutes. No previous temperature excursion involved. No additional adverse event (AE) or product quality complaints (PQC) was reported.
