Symptomtext

Child received vaccines on Wednesday (6/29). Mom reports that she noticed redness and swelling at the left deltoid injection site Thursday night (6/30) and into Friday morning. The redness and swelling progressively got worse Friday (7/1) afternoon and mom was contacted by the child's daycare. Friday evening, the left deltoid injection site started feeling "hot" to the touch. On Saturday (7/2) the redness and swelling did not get any worse. Sunday (7/3) mom reported that the swelling and redness had started to subside. Mom reports no medication was administered for symptoms besides daily Zyrtec.

While taking out Expired inventory. I noticed that Layla Randel received a vaccine 1/17/23 one day after the expiration date 1/16/2023. Pulled records which showed vaccine dose without asterisk. Notified local health dept to verify the status of vaccine. Local Health Dept response. (Notified Staff to contact provider and parent of child regarding non valid vaccine given.) Any expired vaccine needs to repeated even if it was only expired by 1 day. Since Proquad (MMRV) is a live vaccine, it should be repeated atleast four weeks (28 days) after expired dose was given. I will mark the expired dose as invalid in records. Please let me know if you have any questions. Have a great week!

Pertinent medical history, concomitant medication and drug reactions/allergies were not reported. On 04-SEP-2018, the patient was vaccinated with the first dose of varicella vaccine (manufacturer unknown) and measles (+) mumps (+) rubella vaccine (MMR) (manufacturer unknown). On 29-NOV-2021, the patient received the second dose of varicella vaccine (manufacturer unknown) and measles (+) mumps (+) rubella vaccine (manufacturer unknown). On 28-JUN-2022, the patient was vaccinated with a dose of measles, mumps, rubella and varicella (oka-merck) virus vaccine live (rHA) (PROQUAD) injection 0.5 mL (lot# U022728, expire date 16-JAN-2023) inadvertently (strength, route of administered \were not reported) for prophylaxis (accidental exposure to product).
