Symptomtext

Patient developed a diffuse, erythematous, confluent macular rash on the face, chest, abdomen, arms, and legs 12 days after vaccine administration. Rash was not pruritic. No pustules or vesicles. Associated mild congestion. No cough, vomiting, diarrhea, fever. No treatment at home. Recommended topical emollients for comfort and the use of benadryl if itching developed.

Caller stated no adverse side effects have been reported from the patient. No additional AEs reported. No PQCs reported. ? Na; This spontaneous report as received from a medical assistant refers to a patient of unknown age and gender. There was no information about the patient's concurrent conditions, concomitant therapies or medical history provided. On 22-AUG-2022, the patient was vaccinated witha a dose of improperly stored measles, mumps, and rubella (wistar ra 27-3) virus vaccine, live (M-M-R II) lot # U022985, expiration date 22-JUL-2023, albumin status has been verified as recombinant human albumin (rHA), 0.5 mL, subcutaneously (anatomical location was not reported) for prophylaxis. Administered dose of vaccine has been stored in the temperature of -12.3 degrees Celsius (C) for 1 hour (LLT: Product storage error temperature too low). There were no previous temperature excursions reported. No adverse event was reported.

The patient's medical history, concurrent conditions, and concomitant therapies were not reported. On 08-AUG-2022, the patient was vaccinated with 1 dose of improperly stored measles, mumps, and rubella (wistar ra 27-3) virus vaccine, live recombinant human albumin (rHA) (M-M-R II) subcutaneous injection, (0.5 ml, lot number U022985 which was determined to be valid with expiration date: 22-JUL-2023) subcutaneously for prophylaxis (product storage error). The vaccine was exposed to a temperature of -12.3 Celsius degree for a time frame of 1 hour. No previous temperature excursion was provided. No additional adverse event (AE) information was provided.

Symptomtext

Patient became upset and started flailing her arms after DTaP vaccination was administered. Unable to safely administer MMR or Varicella vaccines due to patient being upset and inconsolable, vaccines were wasted with another nurse and provider present.
