Symptomtext
28 yo M with history of liver transplant was vaccinated with Pneumovax 23 IM in right deltoid on 4/8/22 at 0850 the end of his clinic visit. Lot # U023826 (NDC 0006-4943-01). When the patient was at the checkout desk and awaiting labs, he was experiencing shortness of breath and feeling like throat was closing. Reportedly cyanotic. RRT called. Found patient sitting in lobby awake, alert, and flushed. Pt reported sensation as though his throat was closing. VS at 0857- BP 147/103, HR 92, respirations 26, pO2 91% on RA. Patient was placed on 4L O2. Patients stated his head felt like it was on fire, chest and back was hurting. Per reporter, epinephrine was ordered but not given. Patient symptoms started to resolve. VS at 0911- BP 151/100, HR 68, respirations 22, POX 96% on 4l O2. O2 removed and POX remained stable 94-96% on RA. Patient declined to go to emergency room. AMA forms signed with emergency staff. No SOB at 0923 or 0928. Allergy added to patient's profile by Medical staff.
