Symptomtext

patient received MMR, VAR and HAV on 4/22/22. Patient had rash / lesion on right lateral upper thigh at injection site for VARIVAX- noted 5/1/22. Fever to 103 noted 5/1/22 after the rash / lesion started. Patient with significant fussiness during this. Lesion was darker in redness, almost purple with 2 non vesicular papules on either side of the injection site. Patient continued with fever to 101 x 36 hours and defervesced. Injection site lesion improving 5/4/22 at office visit. 5/4/22 evaluation with mild pink, flat blanching 2.5 x 1.5 cm lesion on upper lateral right thigh. No tenderness, no vesicles or papules. No subcutaneous tissue changes. No streaking or warmth. No other skin rashes or change.

3/2/2022 around 8pm started to complain of feeling itchy around her neck - progressed to hives - woke up at midnight with itchy raised rash. Have benadryl at 8:30am 3/3/2022 and rash started to fade.

On 27-OCT-2023, the patient was vaccinated with an expired dose of varicella virus vaccine live (oka/merck)(VARIVAX), 1 dose, (strength, dose number, route, anatomical location, and frequency were not reported), lot # U025789, expiration date 19-AUG-2023, as prophylaxis (expired product administered). No adverse event was reported. It was confirmed product had been stored appropriately.

No side effects reported; Improperly stored vaccine was administered.; This spontaneous report has been received from a nurse concerning a patient of unknown age and gender. The patient's concurrent conditions, medical history, concomitant therapies and drug/allergy history were not reported. On 21-JUN-2023, the patient was vaccinated with an improper stored dose of varicella virus vaccine live (oka/merck) (VARIVAX), lot # U025789 (expiration date reported and upon internal validation established as 19-AUG-2023) for prophylaxis (dose, anatomical location, and route of administration were not reported). The vaccine was stored at a temperature of 6 Celsius (C) degrees for 7 hours. It did not have previous temperature excursions. No side effects were reported.

Nurse (RN) gave Varicella vaccine to patient per providers order (08/21/2023, 10:00am). Did not document before administration. When nurse went to document vaccine, she noted that it expired on 08/19/2023 Varicella: U025789 exp: 08/19/2023 Writer called patient who reports she is feeling her normal self at this time. Reviewed normal side effects of vaccine (pain/minor swelling at injection site, fever) as well as when to seek emergency medical care(severe side effects, feeling very sick, signs of allergic reaction).

Client came in 09/07/2022 to receive a Hepatitis B Peds, DTAP, Polio, Pneumoccoal 13, Hib, MMR; instead he received the Hepatitis B Peds and Varicella. I wrote the clients sibling vaccines on the consent form that we write vaccines on of the above-mentioned vaccines. He did not need a Varicella vaccine he was already up to date. Once I realized that I made this error I called to speak to the parent on 9-8-2022 but his phone number went straight to a recording stating ?this caller phone is not setup to accept voicemail?. I then reached out to the translator that accompanied the family and she answered the phone. She stated that the parents will not answer their phones that is just what they do and that she will be the best contact person. I explained to her that I did give the wrong vaccine and she was familiar with this error because she had an appt slip that had written down what the sibling and this client were to receive. But I was looking at the vaccines that I had written down on the consent form. I asked if she knew if the client had a pediatrician so I could notify their physician of my error and get guidance due to the incorrect vaccine ? Varicella this client received. She stated that this client does not have a pediatrician and that this is the reason the client was brought to the health department. I explained that I do need to get an update on how the child is doing. Also, my error was reported to Nurse Manager on 9-8-2022 and that a VAERS will be done. I explained to her the parent should expect to be contacted by VAERS of this however, I will be entering both the parent phone number and hers on the VAERS, just in case the parent is not reachable. She said ok. She called me this morning 9-9-2022 and stated the parents were very upset and I told her I do not blame them and I apologized several times for my error. She stated the client was up and playing around with no issues. Per translator the parents will be taking the client to a physician to be evaluated and go from there. I did ask that the translator call me back with the physician?s name and she said ok. When I spoke with the translator 9-14-2022 she said that the parents have made an appt with a physician but it is a month out before the child will be seen. I asked that the translator call me back with the md name and date of appt. She said she would but I have not heard back from her as of yet.

There was no information regarding the patient's medical history, concurrent conditions and concomitant medications provided. On 27-JUL-2022, the patient was vaccinated with varicella virus vaccine live (Oka/Merck) (VARIVAX) injection, lot # U025789 confirmed to be valid with an expiration date 19-AUG-2023 (exact dose, route of administration, anatomical location were not reported) for prophylaxis. The vaccine underwent a temperature excursion of - 13.994 degrees Celsius (C) for 1 hour and 20 minutes (product storage error). There was no previous temperature excursion. No additional adverse event was reported.
