Symptomtext

Patient parent called and notified office of seizure 05/25/2022. Went to outside ER for treatment 05/25/2022. Positive for Covid 05/25/2022. Sent home. Told to give Tylenol and Ibuprofen rotating.

Symptomtext

At approx. 2:00 PM after receiving vaccines the patient was walking down the hallway with grandpa and suddenly went down to the floor. The MA called for my help and I got a wheelchair and we assisted the patient back into the exam room and on to the exam table. Vital signs were being taken when Dr. and RN nurse manager) came to evaluate the patient. Dr. ordered a glucose to be done at that time. Vitals and glucose were normal, patient was monitered approx. 15-20 minutes longer and then discharged home with her grandpa.

Symptomtext

Left thigh day after vaccination had two areas of redness which resolved over a few days. Then 9 days after vaccination (6/22/2022) right thigh (site of varicella) developed 3 nodules, worsening and red the next day, then today (6/24/2022) a 6 cm x 3 cm area of erythema, warmth, and induration surrounding the nodules, with enlarged right inguinal lymph nodes. No fluctuance, vesicles, or discharge.

What happened was the MA drew up Proquad, Proquad diluent, and Varivax (all on one syringe) and it was administered to the patient. A double check was done by another MA but not until they were already drawn up. At that point it was labeled MMRV so she thought it was Proquad. I contacted someone @ 1413. I told her the situation and she said that the patient got an extra dose of varicella. She advised we call the parents and discuss the following: There is an increased risk of febrile seizures with the additional dose of varicella. Watch for fevers and give acetaminophen and/or ibuprofen to reduce fevers as needed. MA will contact the patients parent and discuss.
