pt had syncopal event following tdap and varivax administration. she was okay during tdap shot and immediately after safety mechanism activated with varivax, she experienced her event. she pasted out for no more than 5 seconds. She slumped back into her chair without sustaining injury. i was able to lift her upright and she immediately regained consciousness. she was pale and confused at first and i was able to orientate her to person place and time. i administered cold compress and cold water. code was called to pharmacy. in discussion following event, she told me she did not eat much for lunch and had just started her new job. I had a tech grab her gatorade to give hydration, sucrose and electrolytes. she regained her pre-vaccine color. we all suggested she remain in pharmacy for at least 30 minutes, walk around the store for 5 minutes to ensure no repeat events and instructed her to have a family member come support her leaving. her aunt lives close and she called her to tell her she is coming to expect her arrival. she left on her own

mother stated that on Friday 6/3/22 she first noted a "rash" around the injection site. 3 or 4 raised bumps, like bug bites, solid no fluid, not crusted located next to the injection site on the right arm. mom stated child stated they "hurt" when she touched them while applying any pressure to the area. no fever, no fine rash, no pain with just touch without pressure to the area. some itching to the area per mother. this morning 6/6/22 mom noted the bumps are much smaller, "going away" and pt. stated it no longer hurt when she applied pressure to the area. no longer itchy. mom did not seek medical evaluation but waited until this morning to call me. I advised mother if pt. develops a fever or the bumps get bigger or spread to call UC and have a doctor evaluate immediately. but since they were going away to continue to monitor, take pictures in AM and PM for evaluation by MD in future if needed. suggested to mother it could have been a mild allergic reaction and in the future she should go to a UC for evaluation and not wait for me to return to the office for the weekend. Pt. had full energy and did not complain of other issues. mother declined referral to regular physician or UC at this time since bumps are smaller and pain is gone. mother stated brother had one bite on his stomach the same day but it is not longer there. I advised mom it was impossible to evaluate if the bump is gone and that one bump doesn't constitute a rash or reaction since neither child had fever. not a contraindication for vaccination in future. mother is vaccine hesitant and getting only 4 - 6 vaccines per year at this time. child is not in public school at this time.

BUMP AND REDNESS TO LEFT THIGH THE NEXT DAY AFTER RECEIVING VACCINATIONS. ADVERSE REACTION TO VACCINE. CELLULITIS AND INFECTION TO LEFT THIGH. FOLLOW UP NEXT DAY MOM STATES THE BUMP IS STILL THERE BUT IT IS NO LONGER RED AND DOES NOT BOTHER THE PATIENT. DR STATES IMPROVED CONTINUE ANTIBIOTICS RETURN IF NOT IMPROVED. AUGMENTIN 4.5 MLS BY MOUTH 2 TIMES DAILY FOR 10 DAYS

24 hr post vaccination parent reported visible redness, to bilateral thighs. Parent reported warm to the touch and painful. Pain lasted q 24hr and reduce to only tender feeling 48hr post injection. Right leg redness present from below injection site on anterior lateral thigh extending almost to the hip and down to the knee cap. Left leg redness occurred below the knee extending towards her ankle

No additional AE; T/E. Improperly stored VARIVAX administered to a patient on 28AUG2023, 0.5mL dose. HCP did not have information (lot number, expiration date) on DILUENT but confirmed it was Merck DILUENT; This spontaneous report was received from a Medical Assistant and refers to a 6-year-old patient of unknown gender. The patient's medical history, concurrent conditions and concomitant therapies were not provided. On 28-AUG-2023, the patient was vaccinated with varicella virus vaccine live (oka/merck) (VARIVAX) injection, 0.5 milliliter (once), lot #W039294, expiration date: 11-DEC-2024 or lot# U027947, expiration date: 08-SEP-2023, administered for prophylaxis (anatomical location, strength, and route of administration were not provided); and with sterile diluent (no details reported), and the vaccine was improperly stored: temperature 7.8F (6 F degrees up to 14 F degrees), time frame: 0 hour 30 minutes 0 seconds (no previous temperature excursion). No additional adverse event.

No abnormal signs or symptoms have been reported for this patient; She reports this was a vaccine selection error as the provider intended to administer MMR vaccine instead of Proquad; She reports this was a vaccine selection error as the provider intended to administer MMR vaccine instead of Proquad; patient received 0.5 ml of Varivax and 0.5 ml of Proquad due to vaccine selection error; This spontaneous report was received from a medical assistant and refers to a 12-month-old female patient. The patient's concurrent conditions, concomitant therapies and medical history were not provided. On 12-APR-2022, the patient was vaccinated with varicella virus vaccine live (oka/merck) (VARIVAX) dose of 0.5 ml, subcutaneously in the left thigh, lot # U027947, expiration date was reported as 12-APR-2023 but upon internal validation process it was established as 08-SEP-2023 and measles, mumps, rubella and varicella (oka-merck) virus vaccine live recombinant Human albumin (rHA) (PROQUAD) dose of 0.5 ml, subcutaneously in the right thigh, lot # U029324, expiration date 19-MAR-2023, albumin status has been verified as recombinant human albumin (rHA). Both vaccines were administered for prophylaxis. This was a vaccine selection error as the provider intended to administer measles, mumps, and rubella (wistar ra 27-3) virus vaccine, live (M-M-R II) instead of measles, mumps, rubella and varicella (oka-merck) virus vaccine live (PROQUAD). Administration error was discovered on the evening of 12-APR-2022 (also reported as on 13-APR-2023, discrepant information) after the patient was discharged to home with her parents. No abnormal signs or symptoms have been reported.
