Pt appeared nervous during vaccination, but appeared to tolerate well for about a minute or two. Pt advised to remain seated. Pt lost consciousness, tensed her muscles and began to shake. This lasted about 10 seconds. Her mother and myself touched and aroused pt. Advised pt to remain seated another 15 minutes. Pt reported feeling dizzy and sweaty. Pt remained alert and oriented and was able to walk out of office after resting 15 minutes. Pt sat in waiting room during check out. She reported feeling okay and was able to walk out with her mom.

The patient had a near syncopal episode after receiving the HPV vaccine where he slumped in the chair. It was the third injection out of three vaccines he received. the other two were HAVRIX and VARIVAX. His mother and I were able to catch him and hold him upright in the chair. He came around after jerking himself back to consciousness. I gave him a sucker and water and place a wrapped iced pack at the nape of his neck. His skin was sweaty and warm during the episode. I called someone to the room to translate to the mother and the patient the pathophysiology behind the reaction and that it is common with this vaccine. The mother was given the brochure to report the episode. She was told that I would be reporting the episode, however. The patient became nauseated and vomited x 2 during the explanation of the event to the Mom. After sitting and drinking water he recovered after approximately 10 minutes. He refused crackers after sitting for 5 more minutes and the nausea passed. He was fully alert and oriented upon recovery. He was able to ambulate to the waiting area without assistance where he waited an additional 10 minutes. The patient left without further incident or complaint. He had a steady gait upon discharge.

Pt arrived to the country approximately 7 days ago. As per mom patient has no immunization record. Adm. Hep B, IPV, Var, and Tdap to left arm, with no adverse reaction. Administered MCV4 to right deltoid, pt had an episode to syncope followed by a seizure that lasted one minute. Pt awake, alert and oriented X3. States she felt nausea and vomited. Pt was pale and diaphoretic. 11:30, laid pt down and elevated lower extremities. Pt's face and lip color returned to her normal. States she feels fine and requested to sit up. Mother requested for the last two vaccines to be administered. Mom states pt always faints with vaccines and the sight of blood. Administered MMR and HPV to right arm with no adverse reaction. Pt states she felt fine. Instructed mom to go to the ER in case of emergency. Mother verbalized understanding.

Nurse Practitioner and Doctor were notified and patient was evaluated by the providers. Patient blood pressure was checked and patient was given water and instructed to lie down on table. Patient stayed in office for extra 15 minutes.

Caller states the patient was ordered a Flu vaccine but she administered a GARDASIL 9 because that is what she usually administered; but she administered a GARDASIL 9 because that is what she usually administered; This spontaneous report was received from a medical assistant referring to a 14 year old patient of unknown gender. No information regarding the patient's pertinent medical history, drug reactions or allergies and concomitant therapies was provided. The patient was ordered a flu vaccine (indication, dose, route of administration, vaccination scheme frequency, anatomical location, lot number and expiration date were not reported). However, on 09-SEP-2022, instead of the flu vaccine, the patient was inadvertently vaccinated with the third dose (as the patient already completed a 2 dose series) of hpv rl1 6 11 16 18 31 33 45 52 58 vlp vaccine (yeast) (GARDASIL 9) syringe injection (lot # u029033, expiration date:13-MAR-2024) at a dose of 0.5 milliliter, intramuscular as prophylaxis (anatomical location not reported) (wrong product administered), because that is what the reporter usually administered (product confusion).
