After receiving 4 vaccines, patient reportedly passed out while waiting for copy of vaccine record. Code called by front staff at 1342, and medical staff responded to scene immediately with crash cart. Upon arrival, patient was standing up next to father and brother alert. Patient was diaphoretic, saturated from his urine, and stated "I'm okay." Patient brought back to clinic room with father and brother for further assessment. Dr. reported to clinic room and assessed patient. Dr. reported patient lungs clear and normal heart sounds. Initial vital signs: BP 119/82, HR 79, RR 22 breaths/min, pulse ox 99% on room air. Patient given apple juice and crackers. Repeat vital signs at 1354: BP 74/47, HR 61; RR: 18/min. Patient stated feels sick to his stomach and dizzy. Patient drank to small cups of water and ate another cracker. Repeat vitals at 1403: BP 104/75, HR 75, RR 18/min. Patients states "I'm feeling good now" and denies feeling dizzy or sick to his stomach. Patient is alert and oriented X3 and conversing with father and myself. Patient escorted to car with family at 1411 and they were instructed if further syncopal episodes to call EMS and to call our clinic if they have any questions or concerns. Family verbalized understanding.

40 mins after vaccine administration, family was out driving looking at houses and mother noticed that pt. appeared like she fell asleep in her carseat however she appeared like she was staring with eyes crossed, she was breathing, arms were stiff, no cyanosis. This episode lasted about 30 seconds and pt. gradually returned to baseline 2 minutes from the start of the episode. Parents immediately returned to clinic to be evaluated. She has history of breatholding spells, but this instance was different than her previous episodes and last time she had breathholding spells was 8/10 and none since. She didn't have fever, she received Ibuprofen prior to vaccines as she has done in the past.
