Crying Death Inappropriate schedule of product administration

Symptomtext

Post-vaccine visit when reviewing paperwork, determined gave polio vaccine off schedule. Prior polio 12/14/22, 12/15/2020 and 10/01/2020. At time of visit mother had agreed to polio vaccine along with other vaccines given. When called to inform mother of vaccine being off schedule, father answered. Explained to father that child had been given polio off schedule. Father stated child died the night he received vaccination but did not say how. Did not press for any further information. At time of appointment, no contraindications to vaccine noted. Mother had difficulty holding child still during vaccination; however, vaccines were still able to be given. Child cried during vaccination but calmed down and was walking around office post vaccination.

Erythema Malaise Posture abnormal Snoring

Symptomtext

Patient notified staff of past passing out with blood draws. Since patient was in the readiness lab he was placed in a reclined position for safety. Blood was drawn with no issues. patient was then sat up per his request. Patient received Hep B and then JEV with no issues. When his MMR was given with no issues. Patient was then sitting there and then got up about 45 seconds after vaccines patient reported feeling not well. Patient was assisted to set back in chair in a reclined position. Patient's neck hyper-extended, face turned red, patient made a snoring sound. Some thrashing and tense body posturing was noted. After a sternal rub he appeared to be out for about 45 seconds. Ice was applied. Patient woke and spoke. Appropriate, oriented and was able to observe all questions. Patient was resting comfortably, drinking water and observed for about 1 hour. Discharged home with no issues by Clinical Staff

Symptomtext

Began vomiting around 9:30pm on 9 Jan. Became lethargic. Vomited 5 times in a one hour period. Was taken to a local Hospital ER. Abdominal Ultrasound preformed which showed intussusception. Fluid enema performed under XRay which reversed the intussusception. Patient was monitored in hospital for next 48 hours.

Product expired on 23SEPT2023 and was administered on 02NOV2023.; This spontaneous report was received from a physician and refers to a patient of unknown age and gender. The patient's medical history, concurrent conditions, and concomitant therapies were not reported. On 02-NOV-2023, the patient was vaccinated with an expired dose of measles, mumps, and rubella (WISTAR RA 27-3) virus vaccine, live (M-M-R II) injection, therapy type: rHA, valid lot #U029793, expiration date: 23-SEP-2023, administered for prophylaxis (strength and route of administration were not provided) (Expired product administered). HCP calling to report an AE regarding the MMR II vaccine. HCP reported that a patient was administered an improperly stored vaccine. Permission was provided to contact the HCP. AE case 02425114 filed. Case 02425105 filed. No additional AE/PQC rep; This spontaneous report was received from a nurse and referred to a patient. The patient's medical history, concurrent conditions and concomitant therapies were not reported.
