Diarrhoea Rash Swelling face

Symptomtext

Rash to face, arms and some on abdomen, none noted around injection sites. Also reported diarrhea. Mother gave benadryl after noted some rash, swelling to face early am on the morning after vaccination with 12 month vaccines, MMR, Varicella, Hib and Hep A . No reported respiratory distress, wheezing or nausea or vomiting. Instructed by provider to administer benadryl every 6 hours for the next 48 hours, and rtc or call 911 if noted wheezing, hives, worsening diarrhea. Today 5/12/22 Mom states child still has rash to face, no respiratory symptoms reported.

No additional AE; Nurse reporting a temperature excursion for VARIVAX that occurred on 10/10/2022 states a dose has been administered. No additional information provided. No additional AE/PQC Names of vaccines involved: VARIVAX: U032852 10/20/2023 ? Tempe; This spontaneous report was received from a nurse referring to a patient of unknown age and gender. The patient's medical history, historical drugs, concurrent conditions and concomitant medications were not provided. On 10-OCT-2022, the varicella virus vaccine live (oka/merck)(VARIVAX) lot # U032852, expiration date 20-OCT-2023 had a temperature excursion of 6.8 degrees Fahrenheit for 30 minutes, and on 22-OCT-2022 0.5 milliliters were administrated to a patient (product storage error). No adverse event was reported. There were no previous excursions detected.

No additional AE reported; Customer reported that MMR II and VARIVAX were administered to a 2 month old baby in error today (09/13/2022). Patient was not due for these vaccines until she was 12 months; This spontaneous report has been received from a nurse concerning a 2-month-old female patient. The patient's concurrent conditions, medical history, concomitant therapies and drug/allergy history were not reported. On 13-SEP-2022, the patient was vaccinated with measles, mumps, and rubella (wistar ra 27-3) virus vaccine, live (rHA) (M-M-R II), 0.5 milliliter (ml) (lot # U006486, has been verified to be a valid lot number for measles, mumps, and rubella [wistar ra 27-3] virus vaccine, live [rHA] [M-M-R II], expiration date reported and upon internal validation established as 26-FEB-2023) (anatomical location and route of administration were not reported) and varicella virus vaccine live (oka/merck) (VARIVAX) (lot # U032852, has been verified to be a valid lot number for varicella virus vaccine live [oka/merck] [VARIVAX], expiration date reported and upon internal validation established as 20-OCT-2023) (anatomical location and route of administration were not reported).
